INDICATION

Orelox 100mg Tab is a third-generation oral cephalosporin antibiotic indicated for the treatment of mild to moderate bacterial infections caused by susceptible organisms, including community-acquired pneumonia, acute exacerbations of chronic bronchitis, acute sinusitis, pharyngitis and tonsillitis caused by Streptococcus pyogenes, uncomplicated urinary tract infections, and skin and soft tissue infections.

MECHANISM OF ACTION

Cefpodoxime proxetil is an oral prodrug that is absorbed from the gastrointestinal tract and rapidly hydrolyzed to the active compound cefpodoxime. Cefpodoxime exerts bactericidal activity by binding to penicillin-binding proteins and inhibiting the final transpeptidation step of peptidoglycan synthesis in the bacterial cell wall, causing cell wall structural defects and lysis. It is stable against many beta-lactamases and has a broad spectrum of activity against both gram-positive and gram-negative organisms.

DOSAGE AND ADMINISTRATION

•      Adults: 100mg to 200mg twice daily depending on infection severity

•      Uncomplicated UTI: 100mg twice daily for 7 days

•      Pneumonia: 200mg twice daily for 14 days

•      Always complete the full prescribed course even if symptoms resolve early

SIDE EFFECTS

Common:

•      Diarrhoea (most common)

•      Nausea and abdominal discomfort

•      Headache

•      Skin rash or urticaria

Less common:

•      Clostridium difficile-associated colitis with prolonged use

•      Elevated liver enzymes

•      Eosinophilia

•      Hypersensitivity reactions including angioedema or anaphylaxis

NOT RECOMMENDED IN

•      Known hypersensitivity to cephalosporins or penicillin’s (cross-reactivity possible)

•      Severe renal impairment (dose interval adjustment required)

•      History of gastrointestinal disease with cephalosporins

•      Not for severe life-threatening infections requiring parenteral therapy

DRUG INTERACTIONS

Antacids containing Aluminium or magnesium and H2-receptor blockers reduce cefpodoxime absorption; take cefpodoxime 2 hours before or after antacids. Probenecid increases cefpodoxime plasma levels by reducing renal tubular secretion. Anticoagulants such as warfarin may have enhanced effect; monitor INR. Food increases the bioavailability of cefpodoxime proxetil; taking with a meal is recommended.
