INDICATION

Opton 40mg Cap is a proton pump inhibitor (PPI) indicated for the treatment of gastro-esophageal reflux disease (GERD), healing and prevention of recurrence of erosive esophagitis, Zollinger-Ellison syndrome, and as part of Helicobacter pylori eradication regimens. It is also used for prevention of NSAID-induced gastric ulcers in at-risk patients.

MECHANISM OF ACTION

Esomeprazole is the S-enantiomer of omeprazole and a selective, irreversible proton pump inhibitor that binds covalently to the H+/K+-ATPase enzyme on the secretory surface of gastric parietal cells after acid activation. By blocking the final step of gastric acid secretion, it produces profound and sustained suppression of both basal and stimulated acid output. As the pure S-isomer, esomeprazole undergoes less first-pass metabolism via CYP2C19 than racemic omeprazole, resulting in higher bioavailability and more consistent intragastric pH elevation across patient populations, including poor and extensive CYP2C19 metabolizers.

DOSAGE AND ADMINISTRATION

•      Erosive esophagitis: 40mg once daily for 4 to 8 weeks

•      GERD maintenance: 20 to 40mg once daily long-term as directed

•      H. pylori eradication: 40mg twice daily with appropriate antibiotics for 7 to 14 days

•      Take 30 to 60 minutes before the first meal of the day; always follow your doctor's instructions

SIDE EFFECTS

Common:

•      Headache

•      Nausea, diarrhoea, constipation, flatulence

•      Abdominal pain

•      Dry mouth

Less common:

•      Hypomagnesaemia with prolonged use (muscle cramps, arrhythmias, seizures)

•      Vitamin B12 deficiency with long-term therapy

•      Clostridium difficile infection risk with prolonged use

•      Fundic gland polyps with prolonged use (generally benign)

NOT RECOMMENDED IN

•      Known hypersensitivity to esomeprazole, omeprazole, or other substituted benzimidazole PPIs

•      Concurrent use with atazanavir or nelfinavir (significantly reduced antiretroviral absorption)

•      Prolonged use without regular clinical reassessment of ongoing need

•      Severe hepatic impairment without dose reduction

DRUG INTERACTIONS

Esomeprazole inhibits CYP2C19 and has less interaction with clopidogrel than omeprazole, making it a preferred PPI in patients on antiplatelet therapy. It reduces absorption of atazanavir, nelfinavir, ketoconazole, and itraconazole by raising intragastric pH. Digoxin absorption may be slightly increased. Methotrexate renal excretion may be reduced by PPIs; monitor for methotrexate toxicity with concomitant use.
