INDICATION

Nuloc 40mg Injection contains esomeprazole, a proton pump inhibitor (PPI) administered intravenously for the short-term treatment of conditions where oral therapy is not possible. It is indicated for the treatment and prevention of rebleeding of gastric and duodenal ulcers following therapeutic endoscopy, gastroesophageal reflux disease (GERD) when oral administration is not feasible, Zollinger-Ellison syndrome, and stress ulcer prophylaxis in critically ill patients. The injectable form provides rapid, reliable acid suppression in hospitalized patients.

MECHANISM OF ACTION

Esomeprazole is the S-isomer of omeprazole, a substituted benzimidazole that irreversibly inhibits the hydrogen-potassium adenosine triphosphatase (H+/K+ ATPase) enzyme system, commonly known as the proton pump, on the parietal cell surface. By binding covalently to the active enzyme, esomeprazole blocks the final step of acid production, reducing both basal and stimulated gastric acid secretion by up to 90%. The S-isomer has higher bioavailability and more predictable pharmacokinetics than racemic omeprazole.

DOSAGE AND ADMINISTRATION

●     For GI bleeding following endoscopic hemostasis: 80mg IV bolus followed by continuous infusion of 8mg/hour for 72 hours, then switch to oral PPI therapy

●     For GERD when oral dosing is not possible: 20 to 40mg IV once daily

●     For Zollinger-Ellison syndrome: 80mg IV every 12 hours, adjusted based on acid output

●     Reconstitute with 0.9% sodium chloride or 5% dextrose as per manufacturer instructions

●     Administer as IV injection over at least 3 minutes or as IV infusion over 10 to 30 minutes

●     Must be administered by trained healthcare personnel only

SIDE EFFECTS

Common:

●     Injection site reactions including pain, redness, and swelling

●     Headache

●     Nausea and flatulence

●     Diarrhea or constipation

Less common:

●     Hypomagnesemia with prolonged PPI use, which can lead to arrhythmias, muscle spasms, and seizures

●     Clostridium difficile-associated diarrhea

●     Increased risk of bone fractures (hip, wrist, spine) with long-term use

●     Acute interstitial nephritis (rare)

●     Vitamin B12 deficiency with chronic use

NOT RECOMMENDED IN

●     Known hypersensitivity to esomeprazole, any substituted benzimidazole, or any component of the formulation

●     Concurrent use with nelfinavir, rilpivirine, or atazanavir due to significant drug interactions

●     Patients with severe hepatic impairment may require dose reduction

DRUG INTERACTIONS

Esomeprazole reduces gastric acidity and may decrease the absorption of drugs requiring acidic pH, including ketoconazole, itraconazole, erlotinib, and iron salts. Concurrent use with clopidogrel may reduce the antiplatelet effect due to CYP2C19 inhibition. Atazanavir, nelfinavir, and rilpivirine concentrations are significantly reduced and concurrent use is contraindicated. Esomeprazole may increase levels of methotrexate, diazepam, and cilostazol. Monitoring is advised with warfarin co-administration.

STORAGE INSTRUCTIONS

Store the dry powder below 25 degrees Celsius. Protect from light. Once reconstituted, use within 12 hours if stored at room temperature or within 24 hours if refrigerated. Do not freeze. Keep out of reach of children.
