INDICATION

Novofax Eye Ointment 6GM is a combination antibiotic ophthalmic ointment indicated for the treatment and prevention of superficial bacterial infections of the eye and its adnexa. It is prescribed for bacterial conjunctivitis, blepharitis, corneal abrasions at risk of secondary infection, and as prophylaxis after minor ocular procedures or foreign body removal. The combination provides broad-spectrum coverage by pairing a polypeptide antibiotic effective against gram-negative organisms with one effective against gram-positive organisms.

MECHANISM OF ACTION

Polymyxin B is a polypeptide antibiotic that acts on the bacterial cell membrane. It binds to the phospholipid components of gram-negative bacterial outer membranes, disrupting their structure and increasing membrane permeability. This causes leakage of intracellular contents and bacterial cell death. It is particularly effective against Pseudomonas aeruginosa, E. coli, and Klebsiella species. Bacitracin is a polypeptide antibiotic that inhibits bacterial cell wall synthesis by interfering with the dephosphorylation of C55-isoprenyl pyrophosphate, a lipid carrier essential for peptidoglycan formation. It is primarily active against gram-positive organisms including Staphylococcus aureus and Streptococcus species. Together, the two antibiotics provide complementary coverage across both gram-positive and gram-negative pathogens.

DOSAGE AND ADMINISTRATION

     Apply a small ribbon (approximately 1cm) of ointment into the lower conjunctival sac of the affected eye(s) every 3 to 4 hours, or 2 to 3 times daily for mild infections

     For nighttime use: apply once at bedtime to supplement daytime eye drop therapy

     Treatment duration: 7 to 10 days or as directed by the ophthalmologist

     Wash hands thoroughly before and after application

     Gently close the eye and roll the eyeball to distribute the ointment evenly

     Vision will be temporarily blurred after application due to the ointment base

SIDE EFFECTS

Common:

     Temporary blurred vision after application

     Mild burning or stinging upon application

     Eyelid irritation and mild itching

Less common:

     Allergic contact dermatitis of the eyelids or periorbital skin

     Sensitization reactions, particularly to bacitracin, which may preclude future use

     Overgrowth of non-susceptible organisms including fungi with prolonged therapy

     Nephrotoxicity with polymyxin B is a systemic concern but negligible with topical ocular use

NOT RECOMMENDED IN

     Known hypersensitivity to polymyxin B, bacitracin, or any component of the formulation

     Viral eye infections including herpes simplex keratitis

     Fungal eye infections

     Deep penetrating ocular infections where topical therapy alone is insufficient

DRUG INTERACTIONS

Polymyxin B and bacitracin applied topically to the eye have negligible systemic absorption, limiting drug interaction potential. Avoid concurrent use with other nephrotoxic or neurotoxic agents if there is any risk of significant systemic absorption (such as application to large denuded areas). Space other ophthalmic preparations at least 5 to 10 minutes apart, applying drops before ointment when using both.

STORAGE INSTRUCTIONS

Store below 25 degrees Celsius. Keep the tube tightly closed after each use. Avoid touching the nozzle tip to any surface to prevent contamination. Use within 4 weeks of opening. Keep out of reach of children.
