INDICATION

Novia 400mg Tablets contain ribavirin, a synthetic nucleoside analog antiviral agent indicated for the treatment of chronic hepatitis C virus (HCV) infection. Ribavirin is always used in combination with other antiviral agents; it is never used as monotherapy. It is prescribed as part of combination regimens with pegylated interferon alfa or with direct-acting antivirals (DAAs) for the treatment of HCV genotypes 1 through 6 in adults and children aged 3 years and above. The duration of treatment varies from 12 to 48 weeks depending on the HCV genotype, viral load, and the specific combination regimen used.

MECHANISM OF ACTION

Ribavirin is a guanosine nucleoside analog that exerts its antiviral effect through multiple proposed mechanisms. It is phosphorylated intracellularly to its active mono-, di-, and triphosphate forms. Ribavirin triphosphate inhibits viral RNA-dependent RNA polymerase, disrupting viral RNA synthesis. Ribavirin monophosphate inhibits inosine monophosphate dehydrogenase (IMPDH), depleting intracellular GTP pools needed for viral replication. It also enhances the host immune response by promoting a T-helper 1 (Th1) over T-helper 2 (Th2) cytokine profile, and induces viral mutagenesis through incorporation into the viral genome, driving the virus toward error catastrophe.

DOSAGE AND ADMINISTRATION

●     Adults weighing less than 75kg: 400mg in the morning and 600mg in the evening (1000mg daily) with food

●     Adults weighing 75kg or more: 600mg in the morning and 600mg in the evening (1200mg daily) with food

●     Children 3 years and above: weight-based dosing as determined by the treating hepatologist

●     Swallow tablets whole with food to improve absorption

●     Treatment duration is determined by the specific combination regimen and HCV genotype

●     Regular blood monitoring is required throughout treatment, including complete blood count every 2 weeks initially

SIDE EFFECTS

Common:

●     Hemolytic anemia, which is the most significant and dose-limiting adverse effect

●     Fatigue and weakness

●     Headache and insomnia

●     Nausea and loss of appetite

Less common:

●     Severe anemia requiring dose reduction or blood transfusion

●     Teratogenicity, causing severe birth defects if pregnancy occurs during or within 6 months of treatment

●     Depression, irritability, and suicidal ideation, particularly when used with interferon

●     Pulmonary toxicity including dyspnea and pneumonitis

●     Pancreatitis and hepatic decompensation in patients with advanced liver disease

NOT RECOMMENDED IN

●     Pregnancy or in female partners of male patients taking ribavirin, due to severe teratogenic risk

●     Women of childbearing potential not using two reliable forms of contraception during and for 6 months after treatment

●     Severe renal impairment (creatinine clearance below 50ml/min)

●     Hemoglobinopathies including thalassemia major and sickle cell disease

●     Severe or uncontrolled cardiac disease, as anemia can worsen cardiac conditions

●     Autoimmune hepatitis

●     Known hypersensitivity to ribavirin

DRUG INTERACTIONS

Ribavirin inhibits IMPDH and may interact with azathioprine and other purine analogs, increasing the risk of myelosuppression. Concurrent use with didanosine is contraindicated due to increased risk of fatal hepatic failure and lactic acidosis. Zidovudine combined with ribavirin increases the risk of anemia. Ribavirin may reduce the anticoagulant effect of warfarin. Antacids containing aluminum and magnesium may decrease ribavirin absorption.
