INDICATION

 

Norvasc-V 5/80mg is a fixed-dose antihypertensive combination tablet by Pfizer, indicated for the treatment of hypertension in adults whose blood pressure is not adequately controlled on either amlodipine or valsartan monotherapy. The combination of a calcium channel blocker and an angiotensin II receptor blocker (ARB) provides complementary and additive antihypertensive effects through two independent mechanisms. MECHANISM OF ACTION

 

Amlodipine blocks L-type voltage-gated calcium channels in vascular smooth muscle cells, causing arterial vasodilation and reduction of peripheral vascular resistance. Valsartan selectively blocks AT1 angiotensin II receptors, preventing angiotensin II from exerting its vasoconstrictive, aldosterone-releasing, and sodium-retaining effects. Together, these two agents target arterial tone and the renin-angiotensin-aldosterone system (RAAS) simultaneously, producing a synergistic and sustained reduction in both systolic and diastolic blood pressure. The combination also reduces left ventricular hypertrophy and improves endothelial function.

DOSAGE AND ADMINISTRATION

 

•      One tablet of 5/80mg once daily at the same time each day.

•      May be taken with or without food.

•      Blood pressure response should be evaluated after 2–4 weeks of treatment.

•      Higher-strength combinations are available for patients requiring further blood pressure control.

•      Do not abruptly stop antihypertensive therapy; consult a physician before discontinuation.

DRUG INTERACTIONS

 

Co-administration of Norvasc-V with other antihypertensives or diuretics produces additive blood pressure lowering. Potassium-sparing diuretics and potassium supplements combined with valsartan increase hyperkalemia risk; serum potassium monitoring is recommended. NSAIDs reduce the antihypertensive effect of valsartan and may worsen renal function. CYP3A4 inhibitors such as ritonavir may increase amlodipine plasma levels. Lithium levels may increase with concurrent valsartan use; lithium monitoring is advised. Grapefruit juice elevates amlodipine exposure and should be avoided.
