INDICATION

Nib 5mg Tab is a third-generation cardioselective beta-1 adrenergic blocker indicated for the treatment of essential hypertension in adults. It is also used in the management of stable chronic heart failure in elderly patients as adjunctive therapy.

MECHANISM OF ACTION

Nebivolol selectively blocks beta-1 adrenergic receptors in the heart, reducing heart rate, myocardial contractility, and cardiac output, which collectively lower blood pressure. Unlike older beta-blockers, nebivolol also stimulates endothelial nitric oxide synthase (eNOS), producing vasodilation through NO-mediated mechanisms. This dual action results in effective antihypertensive activity with a favorable hemodynamic profile, causing less bradycardia and peripheral vasoconstriction than non-vasodilating beta-blockers.

DOSAGE AND ADMINISTRATION

      Hypertension: 5mg once daily, with or without food

      Heart failure: titrated carefully from lower doses under specialist supervision

      The same time each day is recommended for consistent blood pressure control

      Always follow the dosage and monitoring schedule prescribed by your doctor; never stop abruptly

SIDE EFFECTS

Common:

      Headache and dizziness

      Fatigue

      Bradycardia (slow heart rate)

      Nausea

Less common:

      Cold extremities due to peripheral vasoconstriction

      Bronchospasm in asthmatic patients (less likely due to cardioselectivity)

      Sleep disturbances or nightmares

      Erectile dysfunction

NOT RECOMMENDED IN

      Acute decompensated heart failure requiring inotropic support

      Severe bradycardia or second or third-degree AV block without a pacemaker

      Severe asthma or COPD with significant bronchospasm

      Known hypersensitivity to nebivolol or other beta-blockers

DRUG INTERACTIONS

Verapamil and diltiazem combined with nebivolol increase the risk of AV block and bradycardia; avoid concurrent use. CYP2D6 inhibitors such as fluoxetine, paroxetine, and quinidine increase nebivolol plasma levels significantly; dose reduction may be required. Antihypertensives including diuretics and ACE inhibitors have additive blood pressure-lowering effects; monitor closely. Beta-blockers mask tachycardia as a warning sign of hypoglycemia in diabetic patients on insulin.
