INDICATION

Nezkil Suspension 100mg/5ml is an oxazolidinone antibiotic indicated for the treatment of serious infections caused by susceptible gram-positive organisms including vancomycin-resistant Enterococcus faecium (VRE), nosocomial pneumonia caused by methicillin-resistant Staphylococcus aureus (MRSA), community-acquired pneumonia caused by Streptococcus pneumoniae (including multidrug-resistant strains), and complicated and uncomplicated skin and soft tissue infections caused by MRSA and Streptococcus pyogenes. The suspension formulation enables dosing in patients who cannot swallow tablets, including children and debilitated adults.

MECHANISM OF ACTION

Linezolid is a synthetic antibacterial agent of the oxazolidinone class that inhibits bacterial protein synthesis through a unique mechanism. It binds to the 23S ribosomal RNA of the 50S subunit at the peptidyl transferase center, preventing the formation of a functional 70S initiation complex required for the translation process. Because this mechanism differs from all other antibiotic classes, cross-resistance with other antibiotics is not expected. Linezolid is bacteriostatic against enterococci and staphylococci, and bactericidal against most strains of streptococci.

DOSAGE AND ADMINISTRATION

·        Adults and adolescents above 12 years: 600mg (30ml) every 12 hours for 10 to 28 days depending on infection type

·        Children 5 to 11 years: 10mg/kg every 8 hours, maximum 600mg per dose

·        Children birth to 4 years: 10mg/kg every 8 hours

·        Neonates less than 7 days old: 10mg/kg every 12 hours, increasing to every 8 hours by 7 days of age

·        Shake the bottle well before each dose and use the provided measuring device

·        Duration of treatment: 10 to 14 days for skin infections, 14 to 28 days for pneumonia and VRE infections

SIDE EFFECTS

Common:

·        Diarrhea and nausea

·        Headache

·        Vomiting

·        Thrombocytopenia, which is the most significant adverse effect requiring weekly blood count monitoring

Less common:

·        Myelosuppression including anemia, leukopenia, and pancytopenia, especially with treatment duration exceeding 2 weeks

·        Peripheral and optic neuropathy with prolonged use beyond 28 days

·        Lactic acidosis, particularly with extended therapy

NOT RECOMMENDED IN

·        Known hypersensitivity to linezolid or any oxazolidinone antibiotic

·        Concurrent use with monoamine oxidase inhibitors (MAOIs) or within 2 weeks of MAOI discontinuation

·        Patients with uncontrolled hypertension, pheochromocytoma, or carcinoid syndrome

·        Concurrent use with serotonergic agents including SSRIs, SNRIs, and triptans without careful risk-benefit assessment

DRUG INTERACTIONS

Linezolid is a weak, reversible, non-selective MAO inhibitor. Concurrent use with serotonergic drugs (SSRIs, SNRIs, triptans, meperidine, tramadol) may cause serotonin syndrome. Tyramine-rich foods (aged cheese, fermented meats, tap beer) should be limited during treatment to prevent hypertensive episodes. Concurrent use with sympathomimetic agents (pseudoephedrine, phenylephrine) may increase blood pressure. No significant CYP450-based interactions.

STORAGE INSTRUCTIONS
