INDICATION

Nepawell 0.1% Drops are an ophthalmic NSAID suspension indicated for the treatment of pain and inflammation following cataract surgery and for reducing the risk of postoperative macular edema in diabetic patients undergoing cataract extraction. Nepafenac provides targeted anti-inflammatory action within the eye through its prodrug mechanism, delivering potent cyclooxygenase inhibition at the site of inflammation while minimizing systemic exposure.

MECHANISM OF ACTION

Nepafenac is a unique NSAID prodrug that crosses the cornea in its inactive form. Once inside the anterior chamber and vitreous body, it is hydrolyzed by ocular tissue esterases to amfenac, its active form. Amfenac is a potent inhibitor of both COX-1 and COX-2, blocking the production of prostaglandins that drive postoperative ocular inflammation, pain, and macular edema. The prodrug design achieves higher intraocular drug levels with lower surface exposure, reducing corneal toxicity compared to direct-acting ophthalmic NSAIDs.

DOSAGE AND ADMINISTRATION

·        Instill 1 drop in the affected eye(s) 3 times daily starting 1 day before surgery

·        Continue treatment through the day of surgery and for 14 days postoperatively

·        Shake the bottle gently before use

·        Remove contact lenses before application; wait at least 15 minutes before reinserting

·        If using other eye medications, maintain a 5-minute interval between applications

SIDE EFFECTS

Common:

·        Mild burning or stinging upon instillation

·        Eye redness

·        Temporary blurred vision

·        Foreign body sensation in the eye

Less common:

·        Corneal epithelial breakdown and delayed healing

·        Corneal thinning or perforation in susceptible patients

·        Hypersensitivity reactions including eyelid swelling

·        Iritis or anterior chamber inflammation

NOT RECOMMENDED IN

·        Known hypersensitivity to nepafenac, amfenac, or any other NSAID

·        Patients prone to complicated corneal healing

·        Use for more than 14 days postoperatively without medical review

DRUG INTERACTIONS

Concurrent use with topical corticosteroids may increase the risk of corneal healing problems. Avoid concurrent use with other ophthalmic NSAIDs. Systemic NSAID use alongside topical nepafenac has a theoretical additive risk, though systemic absorption is minimal.

STORAGE INSTRUCTIONS

Store at room temperature between 2 to 25 degrees Celsius. Do not freeze. Shake before use. Discard 4 weeks after opening. Keep tightly closed. Keep out of reach of children.
