INDICATION
Nepafen 0.1% Drops are a non-steroidal anti-inflammatory ophthalmic suspension indicated for the treatment of pain and inflammation associated with cataract surgery. Nepafenac is also indicated for the reduction in the risk of postoperative macular edema following cataract surgery in diabetic patients. It is used starting 1 day before surgery, continued on the day of surgery, and for 2 weeks postoperatively.
MECHANISM OF ACTION
Nepafenac is a prodrug that penetrates the cornea rapidly due to its uncharged structure. Once inside the eye, it is converted by intraocular tissue hydrolases to its active metabolite amfenac, a potent inhibitor of cyclooxygenase-1 and cyclooxygenase-2. By blocking prostaglandin synthesis at the site of ocular inflammation, amfenac reduces pain, swelling, and the risk of cystoid macular edema. The prodrug design allows faster corneal penetration and higher intraocular drug concentrations compared to conventional ophthalmic NSAIDs.
DOSAGE AND ADMINISTRATION
· Instill 1 drop in the affected eye(s) 3 times daily, beginning 1 day before cataract surgery
· Continue on the day of surgery and for 2 weeks after surgery
· Shake the bottle well before each use as the formulation is a suspension
· Do not wear contact lenses during treatment
· Space at least 5 minutes between this and other eye drop preparations
· Begin treatment within the prescribed timeframe relative to surgery for optimal benefit
SIDE EFFECTS
Common:
· Mild stinging and burning upon instillation
· Eye redness and irritation
· Blurred vision immediately after application
· Sticky sensation in the eye
Less common:
· Corneal epithelial defects and delayed wound healing
· Corneal thinning or perforation, particularly with prolonged use or in predisposed patients
· Allergic reactions including eyelid edema and pruritus
· Increased intraocular pressure (rare)

Known hypersensitivity to nepafenac or any NSAID
Patients with a history of complicated or prolonged corneal healing
Active corneal epithelial defect

DRUG INTERACTIONS
Concurrent use with topical corticosteroids may increase the risk of corneal adverse events. Topical NSAIDs may slow corneal wound healing. There is a potential additive bleeding risk with systemic anticoagulants, though ocular absorption is limited. Avoid concurrent use with other ophthalmic NSAIDs.
STORAGE INSTRUCTIONS
