INDICATION

Nebra Eye Ointment 5GM is an aminoglycoside antibiotic ophthalmic ointment indicated for the treatment of external infections of the eye and its adnexa caused by susceptible bacteria. It is prescribed for bacterial conjunctivitis, blepharitis, keratitis, and dacryocystitis. The ointment formulation provides sustained drug contact with the ocular surface, making it especially suitable for nighttime application or for use in patients who have difficulty using eye drops.

MECHANISM OF ACTION

Tobramycin binds irreversibly to the 30S ribosomal subunit of susceptible bacteria, causing misreading of the mRNA genetic code and inhibiting protein synthesis. It is bactericidal at concentrations achievable by topical ocular application. Tobramycin has excellent activity against gram-negative organisms including Pseudomonas aeruginosa, Escherichia coli, Klebsiella, and Proteus species, as well as some gram-positive organisms including Staphylococcus aureus and Staphylococcus epidermidis.

DOSAGE AND ADMINISTRATION

·        Apply a small ribbon (approximately 1cm) into the lower conjunctival sac of the affected eye(s) 2 to 3 times daily

·        For severe infections: apply every 3 to 4 hours until improvement, then reduce frequency

·        For nighttime supplementation: apply once at bedtime along with daytime eye drop therapy

·        Treatment duration is typically 7 to 10 days

·        Wash hands thoroughly before and after application

·        Vision may be temporarily blurred after application

SIDE EFFECTS

Common:

·        Temporary blurred vision after application

·        Mild burning or stinging upon application

·        Eyelid itching and mild irritation

Less common:

·        Allergic conjunctivitis or eyelid dermatitis

·        Corneal toxicity with prolonged use beyond recommended duration

·        Overgrowth of non-susceptible organisms including fungi with extended therapy

NOT RECOMMENDED IN

·        Known hypersensitivity to tobramycin or any aminoglycoside antibiotic

·        Viral or fungal eye infections

·        Extended use beyond 10 days without ophthalmologist review

DRUG INTERACTIONS

Avoid concurrent topical use with other aminoglycoside antibiotics to prevent additive toxicity. Systemic aminoglycoside use alongside topical tobramycin may increase the risk of nephrotoxicity and ototoxicity, though ocular absorption is minimal. No significant interactions with other topical ophthalmic agents.

STORAGE INSTRUCTIONS

Store below 25 degrees Celsius. Protect from light. Keep the tube tightly closed after each use. Avoid contaminating the nozzle tip. Use within 4 weeks of opening. Keep out of reach of children.
