INDICATION

 

Nebicare 10mg is a highly selective third-generation beta-1 adrenergic blocker indicated for the management of hypertension and stable heart failure in adult patients. At the 10mg dose, it provides enhanced blood pressure reduction for patients whose hypertension is not adequately controlled on the 5mg dose. Nebicare is particularly suited for patients with hypertension accompanied by heart failure, as nebivolol improves cardiac output by reducing myocardial oxygen demand without compromising renal or peripheral blood flow.

MECHANISM OF ACTION

Nebivolol works by selectively blocking beta-1 receptors in the heart, which helps slow the heart rate, reduce the force of heart contractions, and lower blood pressure. Unlike other beta-blockers that affect multiple receptors, nebivolol also activates beta-3 receptors in the blood vessels, which increases nitric oxide (NO) production. This causes the blood vessels to widen, improving blood flow and reducing resistance. This combination of heart and vessel effects lowers blood pressure smoothly without causing a rapid heartbeat, while also supporting healthier blood vessels and providing additional heart protection.

DOSAGE AND ADMINISTRATION

•      Adults: 10mg once daily at the same time each day.

•      Take with or without food.

•      Blood pressure response should be assessed after 2–4 weeks of therapy.

•      Dose reduction required in patients with severe renal impairment (creatinine clearance <30 mL/min).

•      Do not abruptly stop; taper gradually over 1–2 weeks to prevent withdrawal effects.

NOT RECOMMENDED IN

Nebicare 10mg is contraindicated in patients with severe bradycardia, sick sinus syndrome, significant AV block (2nd or 3rd degree), and decompensated heart failure requiring inotropic support. It should not be used in patients with severe reactive airway disease or poorly controlled asthma due to beta-1 receptor spill-over at higher doses. Caution is required in patients with diabetes mellitus as nebivolol may mask hypoglycemic symptoms. Not recommended during pregnancy unless clearly necessary. Patients with peripheral vascular disease require careful monitoring.

DRUG INTERACTIONS

Concurrent use of Nebicare with other antihypertensives, diuretics, or vasodilators may produce additive blood pressure lowering. Verapamil and diltiazem combined with beta-blockers can lead to severe bradycardia or AV block. Digoxin co-administration may further slow the heart rate and requires monitoring. CYP2D6 inhibitors such as fluoxetine, paroxetine, and bupropion can significantly increase nebivolol plasma concentrations by reducing its metabolism. NSAIDs may reduce the antihypertensive efficacy of nebivolol through sodium retention and vasopressor effects.
