INDICATION
Nabiloc 2.5mg Tab is a selective beta-1 adrenergic blocker (cardioselective beta-blocker) used in the management of hypertension, stable angina, and heart failure with reduced ejection fraction. The 2.5mg dose is the standard starting dose in heart failure management, where beta-blocker therapy must be initiated at low doses and gradually up-titrated.
MECHANISM OF ACTION
Bisoprolol selectively blocks beta-1 adrenergic receptors in the heart with minimal effect on beta-2 receptors in the lungs and peripheral vasculature at therapeutic doses. This reduces heart rate, myocardial contractility, and cardiac output, lowering blood pressure and reducing myocardial oxygen demand. In heart failure, chronic sympathetic activation is harmful; bisoprolol's blockade of this pathway prevents beta-adrenoceptor downregulation, reduces arrhythmia risk, and improves cardiac function over time.
DOSAGE AND ADMINISTRATION
• Heart failure: 1.25mg once daily initially, increasing to 2.5mg after 1 to 2 weeks; titrate slowly toward 10mg/day
• Hypertension and angina: 5 to 10mg once daily
• Take in the morning with or without food
• Never stop abruptly; taper the dose gradually under medical supervision to prevent rebound effects
SIDE EFFECTS
Common:
• Bradycardia (slow heart rate)
• Fatigue and dizziness
• Cold extremities
• Headache
Less common:
• Bronchospasm in asthmatic patients (less likely at low doses due to cardioselectivity)
• Erectile dysfunction
• Sleep disturbances or nightmares
• Hypoglycemia masking in diabetic patients on insulin
NOT RECOMMENDED IN
• Acute heart failure or decompensated heart failure requiring inotropic support
• Second or third-degree AV block without a pacemaker
• Severe asthma or COPD with significant bronchospasm
• Sick sinus syndrome or marked bradycardia
DRUG INTERACTIONS
Verapamil and diltiazem (non-dihydropyridine calcium channel blockers) combined with bisoprolol increase the risk of AV block and bradycardia; generally, avoid. Clonidine: if both are to be stopped, taper bisoprolol first; abrupt clonidine withdrawal with bisoprolol can cause rebound hypertension. Antiarrhythmics (amiodarone) increase bradycardia risk. Insulin and oral antidiabetics: bisoprolol masks tachycardia as a warning sign of hypoglycemia.
