INDICATION

Musidin 4mg is a centrally acting skeletal muscle relaxant indicated for the relief of muscle spasticity in conditions such as multiple sclerosis, spinal cord injuries, and other disorders involving upper motor neuron dysfunction. It helps reduce involuntary muscle contractions, spasms, and associated pain, improving the patient's ability to participate in physical rehabilitation programs. Musidin is effective for both tonic spasms and phasic spasticity, providing flexible dosing options based on symptom severity.

MECHANISM OF ACTION

Tizanidine selectively stimulates alpha-2 adrenergic receptors at the spinal level, inhibiting the release of excitatory neurotransmitters from interneuronal synapses in the spinal cord. This leads to reduced polysynaptic reflex activity and suppression of spinal cord facilitation, which collectively decrease muscle tone. Unlike peripherally acting muscle relaxants, tizanidine preserves voluntary motor function and does not block neuromuscular transmission. Its effect is most prominent on spinal polysynaptic pathways, making it particularly effective for upper motor neuron spasticity.

DOSAGE AND ADMINISTRATION

•      Initial dose: 2mg once or twice daily; titrate by 2–4mg every 3–4 days.

•      Usual effective range: 12–24mg/day in divided doses.

•      Maximum dose: 36mg in 24 hours.

•      Take with or without food at consistent times each day.

•      Taper gradually when discontinuing; do not stop abruptly.

DRUG INTERACTIONS

Co-administration of tizanidine with CYP1A2 inhibitors such as fluvoxamine or ciprofloxacin is contraindicated as it markedly raises tizanidine plasma concentrations, resulting in severe hypotension and potentially life-threatening sedation. Antihypertensives combined with tizanidine produce additive blood pressure lowering. Central nervous system depressants, including opioids and benzodiazepines, intensify sedation.
