INDICATION

Movax 4mg is a centrally acting muscle relaxant indicated for the management of muscle spasticity associated with multiple sclerosis, spinal cord injury, and other neurological conditions. MECHANISM OF ACTION

Tizanidine is a centrally acting alpha-2 adrenergic agonist that selectively stimulates alpha-2 receptors in the spinal cord and supraspinal sites. Activation of these presynaptic receptors inhibits the release of excitatory amino acids, including glutamate and aspartate,e from interneurons in the spinal cord. This reduction in excitatory neurotransmitter activity decreases polysynaptic spinal reflexes and depresses the facilitation of Ia afferent pathways, resulting in a selective reduction in muscle tone and spasticity without producing significant muscle weakness at therapeutic doses.

DOSAGE AND ADMINISTRATION

•      Adults: Starting dose of 2mg; may increase by 2–4mg every 3–4 days to a maximum of 36mg/day. Doses are typically divided into 2–4 doses per day, depending on the spasticity pattern.

•      Take with or without food; consistency in feeding schedule is important as food affects absorption.

•      Use the lowest effective dose to minimize side effects.

•      Do not abruptly discontinue high doses; taper gradually to avoid rebound hypertension.

Not recommended in patients with hypotension or those using antihypertensive medications due to additive blood pressure lowering. Elderly patients are more sensitive to the hypotensive and sedative effects and require conservative dosing.

DRUG INTERACTIONS

Strong CYP1A2 inhibitors,s including fluvoxamine, ciprofloxacin, and oral contraceptives, substantially increase tizanidine plasma concentrations, raising the risk of serious hypotension and sedation; this combination is contraindicated. Concurrent use with antihypertensives, diuretics, or other CNS depressants enhances blood pressure lowering and sedation. Alcohol significantly amplifies tizanidine's CNS depressant effects and should be avoided. Acetaminophen may delay tizanidine absorption when taken together. Rifampicin reduces tizanidine efficacy by inducing CYP1A2-mediated metabolism.
