INDICATION

Motilium-V 10mg Tablets are a dopamine D2 receptor antagonist indicated for the relief of symptoms of nausea and vomiting, epigastric bloating, fullness, early satiety, and gastroesophageal reflux. It is prescribed for functional dyspepsia, gastroparesis (including diabetic gastroparesis), and nausea and vomiting associated with dopaminergic agonist therapy in Parkinson's disease. Domperidone works peripherally and does not readily cross the blood-brain barrier, giving it a favorable profile compared to metoclopramide.

MECHANISM OF ACTION

Domperidone is a peripheral dopamine D2 receptor antagonist that acts primarily at the chemoreceptor trigger zone (CTZ) in the area postrema, which lies outside the blood-brain barrier, and on dopamine receptors in the upper gastrointestinal tract. By blocking D2 receptors in the CTZ, it suppresses nausea and vomiting. In the GI tract, it enhances gastric motility by increasing the duration of antral and duodenal contractions, accelerating gastric emptying, and improving antroduodenal coordination without affecting gastric acid secretion.

DOSAGE AND ADMINISTRATION

·        Adults: 10mg taken 15 to 30 minutes before meals and at bedtime, up to 3 times daily

·        Maximum recommended daily dose: 30mg

·        Elderly patients: start with the lowest effective dose

·        Duration of treatment should be the shortest possible; reassess need after 1 week

·        Swallow tablets whole with water

·        Take before meals to maximize prokinetic benefit

SIDE EFFECTS

Common:

·        Dry mouth

·        Headache

·        Abdominal cramps and mild diarrhea

Less common:

·        QT prolongation and cardiac arrhythmias, particularly at high doses or in predisposed patients

·        Galactorrhea (abnormal breast milk production) and gynecomastia due to increased prolactin levels

NOT RECOMMENDED IN

·        Known hypersensitivity to domperidone or any component

·        Prolactin-releasing pituitary tumor (prolactinoma)

·        Conditions where gastrointestinal stimulation could be dangerous (GI hemorrhage, perforation, or mechanical obstruction)

·        Moderate to severe hepatic impairment

·        Patients with known QT prolongation or those taking other QT-prolonging medications

DRUG INTERACTIONS

CYP3A4 inhibitors (ketoconazole, itraconazole, erythromycin, clarithromycin) significantly increase domperidone plasma levels and are contraindicated. Anticholinergic agents may reduce domperidone's prokinetic effect. Domperidone may reduce the absorption of orally administered drugs requiring gastric acidity for dissolution.
