INDICATION

Misar-H 80mg+12.5mg Tab is a fixed-dose combination of an angiotensin II receptor blocker and a thiazide diuretic indicated for the treatment of essential hypertension in adults. It is prescribed for patients whose blood pressure is not adequately controlled on telmisartan or hydrochlorothiazide monotherapy alone, simplifying treatment with a single once-daily tablet.

MECHANISM OF ACTION

Telmisartan selectively blocks AT1 angiotensin II receptors, reducing vasoconstriction and aldosterone-mediated fluid retention. Hydrochlorothiazide inhibits sodium-chloride reabsorption in the distal convoluted tubule of the kidney, increasing urinary sodium and water excretion and reducing plasma volume. The two mechanisms are complementary and produce additive antihypertensive effects. Hydrochlorothiazide also partially counteracts the potassium-retaining tendency of telmisartan, resulting in a more balanced electrolyte profile.

DOSAGE AND ADMINISTRATION

•      Adults: 1 tablet once daily, preferably in the morning

•      For patients not adequately controlled on telmisartan 80mg alone

•      Take with or without food; maintain regular monitoring of blood pressure and electrolytes

•      Always follow the dosage prescribed by your doctor

SIDE EFFECTS

Common:

•      Dizziness and headache

•      Fatigue

•      Hypokalaemia (low potassium from HCTZ component)

Less common:

•      Photosensitivity (HCTZ component)

Potassium-depleting agents such as corticosteroids and loop diuretics compound hypokalaemia risk from the HCTZ component; monitor electrolytes. Lithium excretion is reduced by HCTZ, increasing toxicity risk; monitor lithium levels carefully. NSAIDs reduce both the diuretic and antihypertensive effects of this combination. Aliskiren combined with telmisartan is contraindicated in diabetic or renally impaired patients. Antidiabetic drug doses may need adjustment as HCTZ can impair glucose tolerance.
