INDICATION

MISAR 20MG TAB 20S is an angiotensin II receptor blocker (ARB) indicated for the treatment of essential hypertension in adults. It is also used to reduce the risk of cardiovascular morbidity in patients with established atherosclerotic cardiovascular disease or type 2 diabetes with target organ damage who are unable to take ACE inhibitors.

MECHANISM OF ACTION

Telmisartan selectively blocks the angiotensin II type 1 (AT1) receptor, preventing angiotensin II from causing vasoconstriction, aldosterone secretion, and renal sodium and water retention. Unlike ACE inhibitors, telmisartan does not inhibit bradykinin breakdown, so it does not cause the persistent dry cough associated with ACE inhibitors. Telmisartan also has partial peroxisome proliferator-activated receptor gamma (PPAR-gamma) agonist activity, which may contribute to additional metabolic benefits including improved insulin sensitivity.

DOSAGE AND ADMINISTRATION

•      Adults: 20mg once daily; may be increased to 40mg or 80mg once daily based on response

•      20mg is the starting dose for patients sensitive to antihypertensives or with hepatic impairment

•      Take at the same time each day with or without food

•      Always follow the dosage and blood pressure targets prescribed by your doctor

SIDE EFFECTS

Common:

•      Dizziness and headache

•      Fatigue

•      Upper respiratory tract infection

•      Back pain

Less common:

•      Hyperkalemia (elevated potassium), especially with renal impairment or potassium supplements

•      Hypotension, particularly on first dose or in volume-depleted patients

•      Elevated creatinine or worsening renal function

•      Angioedema (rare but serious; discontinue immediately)

NOT RECOMMENDED IN

•      Pregnancy (telmisartan causes fetal harm; contraindicated from second trimester)

•      Concomitant use with Aliskiren in patients with diabetes or renal impairment

•      Severe hepatic impairment or biliary obstructive disorders

•      Known hypersensitivity to telmisartan or any excipient

DRUG INTERACTIONS

Potassium-sparing diuretics, potassium supplements, and other RAAS agents increase hyperkalemia risk significantly; monitor serum potassium closely. NSAIDs reduce the antihypertensive effect of telmisartan and may worsen renal function, particularly in elderly or volume-depleted patients. Lithium levels may increase due to reduced renal clearance; monitor lithium levels. Aliskiren combined with telmisartan is contraindicated in diabetic patients and those with renal impairment.
