INDICATION

Mirtazep 30mg is indicated for the treatment of major depressive disorder (MDD) in adults. It is particularly beneficial for patients with depression accompanied by insomnia, poor appetite, and weight loss, as mirtazapine enhances sleep and stimulates appetite. Mirtazep is also prescribed for anxiety disorders co-existing with depression and for patients who have not responded adequately to SSRI or SNRI therapy.

MECHANISM OF ACTION

Mirtazapine is classified as a noradrenergic and specific serotonergic antidepressant (NaSSA). It works by increasing the levels of norepinephrine and serotonin in the brain through blocking presynaptic alpha-2 adrenergic receptors. At the same time, it blocks certain serotonin receptors (5-HT2 and 5-HT3), directing serotonin activity toward the 5-HT1A receptor, which helps improve mood while reducing anxiety and nausea. DOSAGE AND ADMINISTRATION

•      Adults: 15–30mg once daily, preferably at bedtime due to sedating effects.

•      Do not abruptly discontinue; taper the dose gradually under physician guidance.

DRUG INTERACTIONS

Mirtazapine combined with MAO inhibitors poses a severe risk of serotonin syndrome and is absolutely contraindicated. Co-administration with other serotonergic agents, including SSRIs, SNRIs, tramadol, or triptan,s increases the risk of serotonin syndrome. Cimetidine can increase mirtazapine plasma concentrations by inhibiting hepatic metabolism. Carbamazepine and rifampicin, as CYP3A4 inducers, may reduce mirtazapine efficacy and require dose adjustment.
