INDICATION
Mirtazep 30mg is indicated for the treatment of major depressive disorder (MDD) in adults. It is particularly beneficial for patients with depression accompanied by insomnia, poor appetite, and weight loss, as mirtazapine enhances sleep and stimulates appetite. Mirtazep is also prescribed for anxiety disorders co-existing with depression and for patients who have not responded adequately to SSRI or SNRI therapy.
MECHANISM OF ACTION
Mirtazapine is classified as a noradrenergic and specific serotonergic antidepressant (NaSSA). It works by increasing the levels of norepinephrine and serotonin in the brain through blocking presynaptic alpha-2 adrenergic receptors. At the same time, it blocks certain serotonin receptors (5-HT2 and 5-HT3), directing serotonin activity toward the 5-HT1A receptor, which helps improve mood while reducing anxiety and nausea. Additionally, its strong antihistamine (H1) effect contributes to its sedative properties and can also stimulate appetite.
DOSAGE AND ADMINISTRATION
• Adults: 15–30mg once daily, preferably at bedtime due to sedating effects.
• Dose may be increased to 45mg/day after 1–2 weeks if clinical response is inadequate.
• Take with or without food; swallow whole with water.
• Do not abruptly discontinue; taper the dose gradually under physician guidance.
• Elderly patients may require lower initial doses with careful titration.

NOT RECOMMENDED IN
Mirtazep 30mg is contraindicated in patients with known hypersensitivity to mirtazapine. It must not be used concurrently with MAO inhibitors or within 14 days of discontinuing an MAOI. Caution is required in patients with epilepsy, hepatic or renal impairment, cardiac conduction abnormalities, or hypotension. Not recommended in patients with a history of urinary retention, glaucoma, or manic-depressive illness without mood stabilizer coverage. Careful monitoring for suicidal ideation is mandatory in the initial weeks of treatment, especially in younger patients.
DRUG INTERACTIONS
Mirtazapine combined with MAO inhibitors poses a severe risk of serotonin syndrome and is absolutely contraindicated. Co-administration with other serotonergic agents, including SSRIs, SNRIs, tramadol, or triptan,s increases the risk of serotonin syndrome. Alcohol and benzodiazepines may significantly enhance the sedative effects of mirtazapine; patients should avoid alcohol during treatment. Cimetidine can increase mirtazapine plasma concentrations by inhibiting hepatic metabolism. Carbamazepine and rifampicin, as CYP3A4 inducers, may reduce mirtazapine efficacy and require dose adjustment.
