INDICATION

Mionex 5ml Drops are a fourth-generation fluoroquinolone ophthalmic solution indicated for the treatment of bacterial conjunctivitis caused by susceptible strains of gram-positive and gram-negative organisms. It is also used for prophylaxis against bacterial infection before and after ocular surgery, including cataract extraction, corneal procedures, and refractive surgery. Moxifloxacin provides broad-spectrum bactericidal coverage with excellent ocular tissue penetration, and its preservative-free or self-preserved formulations are well tolerated on the ocular surface.

MECHANISM OF ACTION

Moxifloxacin exerts its bactericidal effect by simultaneously inhibiting two essential bacterial enzymes: DNA gyrase (topoisomerase II) and topoisomerase IV. DNA gyrase is responsible for the negative supercoiling of bacterial DNA required for replication and transcription, while topoisomerase IV is involved in the separation of replicated chromosomal DNA during cell division. By targeting both enzymes, moxifloxacin achieves rapid bacterial killing and reduces the likelihood of resistance development. Its 8-methoxy group enhances activity against gram-positive cocci, while maintaining strong gram-negative coverage.

DOSAGE AND ADMINISTRATION

·        For bacterial conjunctivitis: instill 1 drop in the affected eye(s) 3 times daily for 7 days

·        For surgical prophylaxis: instill 1 drop 4 times daily beginning 1 day before surgery and continuing for 4 to 7 days postoperatively

·        Do not touch the dropper tip to the eye, eyelid, or any surface to prevent contamination

·        Remove contact lenses before instillation and wait at least 15 minutes before reinserting

·        If using other ophthalmic preparations concurrently, space applications at least 5 minutes apart

·        Complete the full course of treatment even if symptoms improve earlier to prevent resistance

SIDE EFFECTS

Common:

·        Mild eye irritation and discomfort upon instillation

·        Dry eye sensation

·        Blurred vision for a few minutes after application

·        Altered taste (dysgeusia) from nasolacrimal drainage

Less common:

·        Conjunctival redness (hyperemia)

·        Eye pain and increased tearing

·        Allergic reactions including eyelid swelling and periorbital dermatitis

·        Keratitis with prolonged or inappropriate use

NOT RECOMMENDED IN

·        Known hypersensitivity to moxifloxacin or any other fluoroquinolone antibiotic

·        Viral eye infections including herpes simplex keratitis

DRUG INTERACTIONS

Topical ophthalmic moxifloxacin has minimal systemic absorption, limiting drug interaction potential. If using other eye drops concurrently, maintain at least a 5-minute interval between applications to ensure proper absorption. Avoid concurrent topical use with preparations containing divalent or trivalent cations (magnesium, aluminum, iron) that may chelate fluoroquinolones and reduce their efficacy.
