INDICATION

Mimcipar 30mg is indicated for the treatment of secondary hyperparathyroidism (SHPT) in adult patients with chronic kidney disease on dialysis. It is also used in the management of hypercalcemia associated with parathyroid carcinoma and in patients with primary hyperparathyroidism for whom parathyroidectomy is indicated but clinically inappropriate. Cinacalcet effectively reduces elevated parathyroid hormone (PTH), calcium, and phosphorus levels in dialysis patients, thereby minimizing the risk of cardiovascular complications and renal osteodystrophy. Mimcipar plays a critical role in preserving bone mineral density in patients with mineral metabolism disturbances. It is a first-in-class calcimimetic agent available in Pakistan.

MECHANISM OF ACTION

Cinacalcet acts as a calcimimetic by increasing the sensitivity of the calcium-sensing receptor (CaSR) on parathyroid gland cells to extracellular calcium. This enhanced sensitivity causes a reduction in PTH secretion at lower serum calcium concentrations than would otherwise trigger suppression. The drug binds allosterically to the CaSR transmembrane domain, shifting its calcium-response curve to the left, thereby reducing PTH output without stimulating calcium absorption from the gut or bone resorption. The result is a coordinated reduction in PTH, serum calcium, and phosphorus levels.

DOSAGE AND ADMINISTRATION

•      Initial dose: 30mg once daily with food or shortly after a meal.

•      Dose may be titrated up to 60mg, 90mg, 120mg, or 180mg every 2–4 weeks based on PTH levels.

•      Swallow tablets whole; do not crush or chew.

•      Monitor serum calcium, phosphorus, and PTH regularly during treatment.

•      Do not initiate if serum calcium is below the lower limit of normal.

NOT RECOMMENDED IN

Mimcipar 30mg is contraindicated in patients with serum calcium below normal limits (hypocalcemia) at the start of therapy. It should not be used in patients with known hypersensitivity to cinacalcet. Use with caution in patients with seizure disorders, as hypocalcemia can lower the seizure threshold.

DRUG INTERACTIONS

Cinacalcet is a strong inhibitor of CYP2D6 and can significantly increase plasma concentrations of drugs metabolized by this enzyme, including tricyclic antidepressants, beta-blockers like metoprolol, and antipsychotics such as haloperidol. Co-administration with ketoconazole or other strong CYP3A4 inhibitors increases cinacalcet exposure substantially; dose adjustment may be required. Concomitant use with calcium or Vitamin D supplements may partially counteract the calcium-lowering effect. Patients on cardiac glycosides should be monitored carefully as electrolyte shifts may alter digoxin toxicity thresholds.
