INDICATION

Metoclon Tablets are indicated for the treatment of nausea and vomiting associated with various causes, including chemotherapy, radiotherapy, postoperative states, and gastroparesis. Metoclopramide is also used to manage symptoms of gastroesophageal reflux disease (GERD) and delayed gastric emptying. It helps accelerate gastric motility in patients with diabetic gastroparesis, reducing bloating, nausea, and early satiety. Metoclon is also used as a prokinetic agent to facilitate gastrointestinal transit in diagnostic procedures such as small bowel intubation. Its antiemetic and prokinetic properties make it one of the most versatile agents for upper gastrointestinal symptom management. It provides rapid symptomatic relief when administered correctly.

MECHANISM OF ACTION

Metoclopramide acts as a central and peripheral dopamine D2 receptor antagonist. By blocking dopamine receptors in the chemoreceptor trigger zone (CTZ) of the medulla, it prevents dopamine-mediated nausea and vomiting signals. Peripherally, it enhances the action of acetylcholine on the gastrointestinal smooth muscle, resulting in increased lower esophageal sphincter tone, enhanced gastric motility, and accelerated gastric emptying. Its dual mechanism makes it effective for both antiemetic and prokinetic indications.

DOSAGE AND ADMINISTRATION

•      Adults: 10mg three times daily, 30 minutes before meals and at bedtime.

•      Maximum dose: 30mg daily (0.5mg/kg/day).

•      Use for a maximum of 5 days for acute nausea and vomiting.

•      For gastroparesis: follow the physician's prescribed duration, typically up to 12 weeks.

•      Reduce dose in patients with renal or hepatic impairment.

NOT RECOMMENDED IN

Metoclon is contraindicated in patients with known hypersensitivity to metoclopramide, gastrointestinal hemorrhage, obstruction, or perforation. It must not be used in patients with pheochromocytoma due to the risk of hypertensive crisis. Caution is required in patients with Parkinson's disease, epilepsy, or a history of tardive dyskinesia. Not recommended in children under 1 year of age. Avoid in patients with prolactin-dependent tumors.

DRUG INTERACTIONS

Concurrent use of metoclopramide with antipsychotics or other dopamine antagonists may increase the risk of extrapyramidal reactions. The absorption of drugs taken orally may be affected by changes in gastric motility; for example, the absorption of cyclosporine may be enhanced. Metoclopramide may reduce the efficacy of antiparkinson agents such as levodopa. Opioid analgesics and anticholinergic drugs may antagonize the prokinetic effects of metoclopramide. Alcohol and CNS depressants may potentiate sedative side effects.
