INDICATION

Meroque 500mg Inj is a broad-spectrum carbapenem antibiotic indicated for severe or life-threatening hospital-acquired infections, including complicated intra-abdominal infections, complicated urinary tract infections, pneumonia (including ventilator-associated), meningitis in adults and children, and infections caused by multi-drug resistant (MDR) gram-negative bacteria.

MECHANISM OF ACTION

Meropenem is a bactericidal carbapenem beta-lactam antibiotic that inhibits bacterial cell wall synthesis by binding with high affinity to penicillin-binding proteins (PBPs), particularly PBP 2 and PBP 3. This binding inhibits peptidoglycan cross-linking, leading to cell wall lysis and bacterial death. Meropenem is highly resistant to hydrolysis by most beta-lactamases, including extended-spectrum beta-lactamases (ESBLs) and AmpC enzymes, giving it activity against many multi-resistant gram-negative organisms.

DOSAGE AND ADMINISTRATION

•      Adults: 500mg to 1g every 8 hours by IV infusion over 15 to 30 minutes

•      Meningitis: 2g every 8 hours

•      Renal dose adjustment required based on creatinine clearance

•      Administered by intravenous infusion only in a hospital setting under medical supervision

SIDE EFFECTS

Common:

•      Diarrhoea

•      Nausea and vomiting

•      Rash at infusion site

•      Elevated liver enzymes

Less common:

•      Seizures, particularly at high doses in patients with renal impairment or CNS disease

•      Clostridium difficile diarrhoea with prolonged use

•      Thrombocytopenia or neutropenia

•      Hypersensitivity reactions including anaphylaxis

NOT RECOMMENDED IN

•      Known hypersensitivity to meropenem or carbapenems

•      Caution in patients with penicillin allergy (cross-reactivity rare but possible)

•      Severe renal impairment without dose adjustment

•      Hospital use only; not for self-administration

DRUG INTERACTIONS

Probenecid inhibits renal tubular secretion of meropenem, increasing and prolonging plasma levels; avoid concurrent use unless intentional. Valproic acid levels can be dramatically reduced (by up to 60 to 90%) by meropenem and other carbapenems through unknown mechanisms; substitute anticonvulsants if possible or increase valproate dose with close monitoring. Concurrent nephrotoxic agents such as aminoglycosides increase kidney damage risk.
