INDICATION

Megaklar 250mg/5ml Suspension is a higher-strength macrolide antibiotic formulation indicated for moderate to severe bacterial infections requiring elevated dosing in older children, adolescents, and adults who prefer liquid over tablets. It is prescribed for community-acquired pneumonia, sinusitis, pharyngitis, tonsillitis, skin infections, and as part of triple therapy for Helicobacter pylori eradication.

MECHANISM OF ACTION

Clarithromycin binds to the 50S ribosomal subunit at the peptidyl transferase center, halting bacterial protein synthesis. Its active metabolite, 14-hydroxyclarithromycin, works synergistically against respiratory pathogens. Clarithromycin also accumulates intracellularly within macrophages, achieving tissue concentrations several times higher than plasma levels.

DOSAGE AND ADMINISTRATION

·        Older children and adolescents: 250mg (5ml) twice daily for 7 to 14 days

·        Adults preferring liquid form: 250 to 500mg twice daily

·        For H. pylori eradication: 500mg twice daily with a PPI and amoxicillin for 14 days

·        Renal impairment (CrCl below 30ml/min): reduce dose by 50%

·        Shake thoroughly before measuring each dose

·        Complete the full course even if symptoms improve early

SIDE EFFECTS

Common:

·        Gastrointestinal disturbances including diarrhea, nausea, and abdominal pain

·        Taste distortion commonly described as metallic or bitter

·        Headache and insomnia

·        Mild to moderate elevation of hepatic transaminases

Less common:

·        QT interval prolongation with risk of ventricular tachycardia

·        Severe hepatotoxicity including hepatocellular and cholestatic hepatitis

·        Clostridium difficile-associated diarrhea and colitis

·        Hearing loss, usually reversible upon discontinuation

NOT RECOMMENDED IN

Known hypersensitivity to clarithromycin or any macrolide

Co-administration with ergot derivatives, pimozide, cisapride, or lomitapide

History of cholestatic jaundice with prior clarithromycin use

Patients with uncorrected hypokalemia due to QT prolongation risk

DRUG INTERACTIONS

Clarithromycin is a strong CYP3A4 inhibitor. Simvastatin and lovastatin must not be co-administered. Colchicine toxicity may become life-threatening. Levels of cyclosporine, tacrolimus, sirolimus, carbamazepine, and theophylline are significantly elevated. Digoxin concentrations increase. Warfarin effects are enhanced.

STORAGE INSTRUCTIONS

Store dry granules below 25 degrees Celsius. After reconstitution, store at room temperature. Do not refrigerate. Use within 14 days. Keep tightly closed. Keep out of reach of children.
