INDICATION

Megaklar 125mg/5ml Suspension is a macrolide antibiotic indicated for the treatment of mild to moderate bacterial infections in pediatric patients. It is prescribed for pharyngitis, tonsillitis, acute maxillary sinusitis, community-acquired pneumonia, acute otitis media, and uncomplicated skin and soft tissue infections. Clarithromycin also has activity against Helicobacter pylori and Mycobacterium avium complex infections.

MECHANISM OF ACTION

Clarithromycin exerts its antibacterial effect by binding to the 50S ribosomal subunit at the 23S rRNA component, inhibiting translocation of aminoacyl transfer-RNA and blocking protein synthesis. It is also metabolized to 14-hydroxyclarithromycin, an active metabolite that provides additional antibacterial activity, particularly against Haemophilus influenzae.

DOSAGE AND ADMINISTRATION

·        Children (6 months and above): 7.5mg/kg twice daily for 5 to 14 days

·        For acute otitis media: 7.5mg/kg twice daily for 10 days

·        Maximum daily dose should not exceed 500mg twice daily

·        Shake the bottle well before each dose

·        May be taken with or without food; food may reduce gastric irritation

SIDE EFFECTS

Common:

·        Diarrhea, nausea, vomiting, and abdominal discomfort

·        Abnormal taste (metallic or bitter) during therapy

·        Headache

·        Mild elevation of liver enzymes

Less common:

·        Pseudomembranous colitis caused by Clostridium difficile

·        QT prolongation, particularly in patients with pre-existing cardiac conditions

·        Hepatotoxicity with cholestatic jaundice in rare cases

·        Hypersensitivity reactions including rash and anaphylaxis

NOT RECOMMENDED IN

Known hypersensitivity to clarithromycin or any macrolide antibiotic

Concurrent use with cisapride, pimozide, ergotamine, or dihydroergotamine

History of QT prolongation or ventricular arrhythmia

Severe hepatic impairment combined with renal impairment

DRUG INTERACTIONS

Clarithromycin is a potent CYP3A4 inhibitor. Statins (simvastatin, lovastatin) are contraindicated due to rhabdomyolysis risk. Carbamazepine, cyclosporine, tacrolimus, and theophylline levels increase and require monitoring. Colchicine toxicity risk increases significantly. Warfarin effect is potentiated.

STORAGE INSTRUCTIONS

Store the dry granules below 25 degrees Celsius. After reconstitution, store at room temperature between 15 to 30 degrees Celsius. Do not refrigerate. Use within 14 days. Keep out of reach of children.
