INDICATION
Meditim 0.5% Drops are indicated for the reduction of elevated intraocular pressure in patients with chronic open-angle glaucoma, aphakic glaucoma, and ocular hypertension. Timolol is one of the most widely prescribed first-line agents for glaucoma management and may be used alone or in combination with other IOP-lowering agents.
MECHANISM OF ACTION
Timolol maleate is a non-selective beta-adrenergic receptor blocking agent that reduces intraocular pressure by decreasing aqueous humor production in the ciliary body. It achieves this by blocking beta-2 adrenergic receptors on the ciliary epithelium. The onset of IOP reduction occurs within 30 minutes, with peak effect at 1 to 2 hours and duration of approximately 12 to 24 hours.
DOSAGE AND ADMINISTRATION
· Instill 1 drop of 0.5% solution in the affected eye(s) twice daily, morning and evening
· Maintain consistent 12-hour intervals between doses
· Apply nasolacrimal occlusion for 2 minutes after each dose to minimize systemic absorption
· Monitor intraocular pressure regularly during treatment
· Do not discontinue abruptly without consulting your ophthalmologist
SIDE EFFECTS
Common:
· Mild eye irritation, burning, or stinging upon instillation
· Dry eyes and blurred vision
· Slight headache
Less common:
· Systemic beta-blockade effects including bradycardia, hypotension, and shortness of breath
· Exacerbation of asthma or COPD
· Depression, fatigue, dizziness, and sleep disturbances
· Corneal anesthesia with prolonged use
NOT RECOMMENDED IN
· Bronchial asthma or severe COPD
· Sinus bradycardia, heart block greater than first degree, or cardiogenic shock
· Uncompensated heart failure
· Known hypersensitivity to timolol or any beta-blocker
DRUG INTERACTIONS
Concurrent use with systemic beta-blockers produces additive cardiac effects. Calcium channel blockers may cause AV conduction disturbances and hypotension. Digitalis glycosides combined with timolol may produce additive bradycardia. CYP2D6 inhibitors may increase systemic timolol levels.
STORAGE INSTRUCTIONS
Store at room temperature between 15 to 25 degrees Celsius. Protect from light. Do not freeze. Discard 4 weeks after first opening. Keep the bottle tightly closed. Keep out of reach of children.
