INDICATION
Medisopt Drop is a fixed-dose combination ophthalmic solution indicated for the reduction of elevated intraocular pressure in patients with open-angle glaucoma or ocular hypertension who are insufficiently responsive to beta-blockers alone. It provides the convenience of a single bottle containing two complementary IOP-lowering agents to prevent progressive optic nerve damage and visual field loss.
MECHANISM OF ACTION
Dorzolamide is a topical carbonic anhydrase inhibitor that reduces aqueous humor secretion by approximately 40 to 60% by inhibiting carbonic anhydrase II in the ciliary body. Timolol is a non-selective beta-adrenergic antagonist that further decreases aqueous humor production by blocking beta-2 receptors. The combination provides additive IOP reduction greater than either agent alone, typically lowering IOP by 7 to 10 mmHg.
DOSAGE AND ADMINISTRATION
· Instill 1 drop in the affected eye(s) twice daily, approximately 12 hours apart
· Apply gentle nasolacrimal occlusion for 2 minutes after instillation to reduce systemic absorption
· Remove contact lenses before instillation and wait 15 minutes before reinserting
· If using additional eye drops, space applications at least 10 minutes apart
· Do not alter the dosing frequency without consulting your ophthalmologist
· Continue using even if you feel well, as glaucoma often has no noticeable symptoms
SIDE EFFECTS
Common:
· Burning and stinging sensation upon instillation
· Bitter or unusual taste in the mouth due to nasolacrimal drainage
· Blurred vision immediately after application
· Mild eye redness and superficial punctate keratitis
Less common:
· Systemic beta-blocker effects including bradycardia, hypotension, and bronchospasm
· Fatigue, dizziness, and headache
· Corneal edema in patients with compromised corneal endothelium
· Allergic reactions including eyelid dermatitis
NOT RECOMMENDED IN
Bronchial asthma, severe COPD, or history of bronchospasm
Sinus bradycardia, second or third-degree AV block, or overt cardiac failure
Known hypersensitivity to sulfonamides, beta-blockers, or any component
Severe renal impairment (creatinine clearance below 30ml/min)
DRUG INTERACTIONS
Concurrent use with systemic beta-blockers may produce additive cardiac effects. Oral carbonic anhydrase inhibitors may increase systemic side effects. Calcium channel blockers may potentiate hypotension. CYP2D6 inhibitors may increase systemic timolol exposure.
STORAGE INSTRUCTIONS
