INDICATION

Medigesic Tablet is indicated for the relief of acute musculoskeletal pain, muscle spasms, and discomfort associated with strains, sprains, and back pain. The combination of orphenadrine and paracetamol provides dual-action relief by addressing both the muscular and pain components of musculoskeletal conditions. It is also used as an adjunct in the management of pain caused by orthopedic conditions, postoperative muscle spasms, and tension headaches associated with neck and shoulder muscle stiffness.

MECHANISM OF ACTION

Orphenadrine citrate acts as a centrally acting muscle relaxant by blocking excitatory pathways in the brainstem reticular formation and by its anticholinergic and antihistaminic properties, which reduce involuntary muscle spasms. Paracetamol inhibits prostaglandin synthesis in the central nervous system by acting on cyclooxygenase (COX) enzymes, and may also activate descending serotonergic inhibitory pathways to reduce pain perception. Together, these two agents target the pain-spasm cycle from different angles, providing comprehensive musculoskeletal pain relief.

DOSAGE AND ADMINISTRATION

•      Adults: 1–2 tablets three times daily as needed for pain relief.

•      Do not exceed 6 tablets in 24 hours.

•      Take with food or milk to minimize gastrointestinal upset.

•      Not recommended for continuous use beyond 10 days without medical supervision.

•      Reduce dose in elderly patients or those with hepatic impairment.

DRUG INTERACTIONS

Orphenadrine may potentiate the anticholinergic effects of other drugs including antihistamines, tricyclic antidepressants, and antipsychotics. CNS depressants such as benzodiazepines, opioids, and alcohol may intensify sedative effects when combined with Medigesic. Concurrent use with MAO inhibitors is contraindicated. Paracetamol may interact with warfarin when used regularly, potentially enhancing anticoagulant effects; INR monitoring is advised. Enzyme inducers such as rifampicin may reduce paracetamol efficacy.
