INDICATION

Medigesic Forte Tab is a combination analgesic used for the relief of moderate to severe pain associated with muscle tension, musculoskeletal pain, tension headache, and pain conditions where a skeletal muscle relaxant component is beneficial. It combines the analgesic effect of paracetamol with the CNS-stimulant effect of caffeine and the muscle-relaxing properties of orphenadrine.

MECHANISM OF ACTION

Paracetamol inhibits prostaglandin synthesis centrally in the CNS, reducing pain signaling without significant peripheral anti-inflammatory activity. Caffeine enhances analgesia by increasing the potency of paracetamol through CNS stimulation and antagonism of adenosine receptors, which also reduces the sedative burden of pain. Orphenadrine citrate acts as a centrally acting anticholinergic skeletal muscle relaxant that reduces muscle spasm and stiffness, addressing the muscular component of pain syndromes.

DOSAGE AND ADMINISTRATION

•      Adults: 1 to 2 tablets every 6 to 8 hours as needed

•      Do not exceed 8 tablets (4g paracetamol) in 24 hours

•      Take with water; may be taken with food if stomach irritation occurs

•      Always follow the dosage and duration recommended by your doctor

SIDE EFFECTS

Common:

•      Dry mouth (orphenadrine-related)

Less common:

Propoxyphene used with orphenadrine may cause confusion and tremor. Paracetamol combined with alcohol or hepatotoxic drugs increases liver damage risk. Caffeine may reduce the efficacy of sedative medications.
