INDICATION
Medifen 0.05% Eye Drops are a non-steroidal anti-inflammatory ophthalmic solution indicated for the management of postoperative inflammation following cataract surgery, inhibition of intraoperative miosis during cataract extraction, and treatment of pain and photophobia associated with corneal refractive surgery. It is also used for symptomatic relief of seasonal allergic conjunctivitis.
MECHANISM OF ACTION
Diclofenac sodium is an NSAID that inhibits cyclooxygenase (COX-1 and COX-2) enzymes, blocking the conversion of arachidonic acid to prostaglandins. In the eye, prostaglandins mediate inflammation, miosis during surgery, and disruption of the blood-aqueous barrier. By reducing prostaglandin synthesis locally, diclofenac decreases inflammation and reduces postoperative pain without the immunosuppressive risks associated with corticosteroid eye drops.
DOSAGE AND ADMINISTRATION
· For postoperative inflammation: instill 1 drop 4 times daily starting 24 hours after surgery for up to 2 weeks
· For prevention of intraoperative miosis: instill 1 drop every 30 minutes beginning 2 hours before surgery
· For allergic conjunctivitis: instill 1 drop 4 times daily for the duration of symptoms
· For corneal pain after refractive surgery: instill 1 drop 4 times daily for up to 3 days
· Do not wear contact lenses during treatment
· Space at least 5 minutes between this and any other eye drop
SIDE EFFECTS
Common:
· Transient burning and stinging upon instillation
· Mild eye redness and irritation
· Blurred vision immediately after application
Less common:
· Corneal thinning and corneal melt with prolonged use beyond 2 weeks
· Delayed corneal epithelial healing
· Keratitis and corneal erosion in susceptible patients
· Allergic reactions including eyelid swelling
NOT RECOMMENDED IN
Known hypersensitivity to diclofenac or other NSAIDs
Patients with NSAID-sensitive asthma or history of aspirin triad
Active corneal epithelial breakdown or corneal ulcer
Concurrent use with topical corticosteroids in patients at risk for corneal complications
DRUG INTERACTIONS
Concurrent use with topical corticosteroids may increase the risk of corneal healing complications. Ophthalmic NSAIDs may slow corneal epithelial healing. There is a theoretical increased bleeding risk with systemic anticoagulants, though systemic absorption from eye drops is minimal.
STORAGE INSTRUCTIONS
Store below 25 degrees Celsius. Protect from light. Do not freeze. Discard within 4 weeks of opening. Keep the bottle tightly closed. Keep out of reach of children.
