INDICATION

Medicarpine 2% Eye Drops are indicated for the management of elevated intraocular pressure in chronic open-angle glaucoma and acute angle-closure glaucoma as emergency treatment prior to surgery. Pilocarpine is also used to counteract mydriasis induced by anticholinergic or sympathomimetic agents after ophthalmic examination, and in the management of ocular hypertension.

MECHANISM OF ACTION

Pilocarpine is a direct-acting parasympathomimetic (cholinergic agonist) that stimulates muscarinic receptors in the eye. It causes contraction of the iris sphincter muscle, producing miosis, and contraction of the ciliary muscle, which opens the trabecular meshwork and Schlemm's canal. This facilitates increased outflow of aqueous humor, reducing intraocular pressure. The onset of miosis occurs within 10 to 30 minutes and the pressure-lowering effect lasts approximately 4 to 8 hours.

DOSAGE AND ADMINISTRATION

·        For chronic glaucoma: instill 1 to 2 drops in the affected eye(s) up to 4 times daily

·        For acute angle-closure glaucoma: instill 1 drop every 5 minutes for 3 to 6 doses, then every 1 to 3 hours

·        For reversal of mydriasis: instill 1 drop, repeating after 5 minutes if needed

·        Apply gentle pressure to the inner corner of the eye for 1 to 2 minutes after instillation

·        Remove contact lenses before application and wait 15 minutes before reinserting

·        Maintain consistent dosing intervals for optimal intraocular pressure control

SIDE EFFECTS

Common:

·        Miosis causing temporary dimming of vision, especially in low-light conditions

·        Brow ache and periorbital pain, particularly during the first few days

·        Accommodative spasm leading to blurred near or distance vision

·        Mild eye irritation and stinging upon instillation

Less common:

·        Retinal detachment, particularly in patients with pre-existing retinal pathology or high myopia

·        Increased lacrimation and conjunctival congestion

·        Systemic cholinergic effects including sweating, nausea, salivation, and bradycardia

·        Allergic contact dermatitis of the eyelids

NOT RECOMMENDED IN

Conditions where pupillary constriction is undesirable, such as acute iritis and anterior uveitis

Known hypersensitivity to pilocarpine or any component

Patients at risk for retinal detachment including those with high myopia

Use with caution in patients with asthma

DRUG INTERACTIONS

Concurrent use with other miotics may produce additive effects. Anticholinergic agents directly oppose pilocarpine's action. Beta-blockers and carbonic anhydrase inhibitors provide additive IOP reduction. Echothiophate and other cholinesterase inhibitors may potentiate pilocarpine's effects.

STORAGE INSTRUCTIONS

Store at room temperature between 15 to 25 degrees Celsius. Protect from light. Do not freeze. Discard 4 weeks after first opening. Keep the dropper tip clean and the bottle tightly closed. Keep out of reach of children.
