INDICATION

Mecobal Injection is indicated for the treatment and prevention of vitamin B12 deficiency and its neurological and hematological complications. It is used in peripheral neuropathy including diabetic neuropathy, megaloblastic anemia, pernicious anemia, subacute combined degeneration of the spinal cord, and nerve damage resulting from chronic illness, nutritional deficiency, or prolonged metformin therapy. Mecobalamin is the neurologically active form of vitamin B12, making it particularly beneficial for patients with nerve-related symptoms including numbness, tingling, burning sensations, and weakness.

MECHANISM OF ACTION

Mecobalamin is the methylated, biologically active coenzyme form of vitamin B12 that does not require metabolic conversion before exerting its effects. It serves as an essential cofactor for methionine synthase, converting homocysteine to methionine and regenerating tetrahydrofolate for DNA synthesis. In nerve tissue, mecobalamin promotes the synthesis of lecithin, a key component of the myelin sheath. By restoring myelin integrity and supporting axonal transport, mecobalamin improves nerve conduction velocity and reduces neuropathic symptoms.

DOSAGE AND ADMINISTRATION

·        Standard dose: 500mcg by intramuscular injection 3 times per week for the initial 2 weeks

·        Maintenance phase: 500mcg intramuscularly once weekly, adjusting frequency based on response

·        For severe deficiency with neurological involvement: 500mcg daily for 7 days initially

·        Long-term maintenance: 500mcg monthly once adequate B12 levels are restored

·        Must be administered by a trained healthcare professional

·        Monitor serum B12 levels and complete blood count periodically

SIDE EFFECTS

Common:

·        Pain, swelling, or induration at the injection site

·        Headache following administration

·        Mild nausea

Less common:

·        Diarrhea and loss of appetite
