INDICATION

Mebradex Drops 5ML is a combination ophthalmic preparation indicated for steroid-responsive inflammatory conditions of the eye where bacterial infection or risk of bacterial infection exists. These include bacterial conjunctivitis with associated inflammation, blepharitis, blepharoconjunctivitis, keratitis, iridocyclitis, and post-operative inflammation following ocular surgery including cataract extraction and corneal transplant.

MECHANISM OF ACTION

Tobramycin is an aminoglycoside antibiotic that exerts its bactericidal effect by binding irreversibly to the 30S ribosomal subunit of susceptible bacteria, causing misreading of mRNA and inhibition of protein synthesis. It is particularly effective against gram-negative organisms including Pseudomonas aeruginosa. Dexamethasone is a potent synthetic corticosteroid that suppresses inflammation by inhibiting phospholipase A2, reducing prostaglandin and leukotriene production, stabilizing cellular membranes, and reducing vascular permeability.

DOSAGE AND ADMINISTRATION

·        For mild to moderate infections: instill 1 to 2 drops into the affected eye(s) every 4 to 6 hours

·        For severe infections: instill 1 to 2 drops every 2 hours initially, reducing frequency as the condition improves

·        Typical treatment duration is 7 to 14 days; do not exceed 14 days without ophthalmologist review

·        Wash hands thoroughly before and after each application

·        Avoid touching the dropper tip to the eye, eyelid, or any surface to prevent contamination

·        If using multiple eye medications, wait at least 5 to 10 minutes between applications

SIDE EFFECTS

Common:

·        Temporary blurred vision for 5 to 10 minutes after instillation

·        Mild burning or stinging sensation at the application site

·        Eye redness and mild irritation

·        Tearing and mild discomfort

Less common:

·        Increased intraocular pressure with prolonged corticosteroid use

·        Posterior subcapsular cataract formation with extended therapy beyond 14 days

·        Secondary ocular infection due to suppression of host immune response

·        Allergic contact dermatitis of the eyelids

NOT RECOMMENDED IN

Herpes simplex keratitis (dendritic ulcer) and other viral diseases of the cornea

Vaccinia, varicella, and other viral infections of the eye

Mycobacterial or fungal infections of the eye

Known hypersensitivity to aminoglycosides, corticosteroids, or any component

DRUG INTERACTIONS

Systemic absorption from topical ophthalmic application is minimal. However, concurrent use with other aminoglycoside antibiotics may increase nephrotoxicity and ototoxicity risk. Avoid concurrent use with other corticosteroid eye drops. If using contact lenses, remove them before application and wait at least 15 minutes before reinserting.

STORAGE INSTRUCTIONS

Store between 2 to 8 degrees Celsius. Do not freeze. Protect from light by keeping in the original carton. Discard 4 weeks after first opening. Keep the cap tightly closed after each use. Keep out of reach of children.
