INDICATION

Maxpan 100mg Suspension is a third-generation cephalosporin antibiotic indicated for the treatment of mild to moderate bacterial infections in pediatric and adult patients. It is prescribed for acute otitis media, pharyngitis and tonsillitis caused by Streptococcus pyogenes, uncomplicated urinary tract infections caused by Escherichia coli and Proteus mirabilis, acute exacerbations of chronic bronchitis caused by Streptococcus pneumoniae and Haemophilus influenzae, and uncomplicated gonorrhea caused by Neisseria gonorrhoeae. MECHANISM OF ACTION

Cefixime exerts its bactericidal effect by binding to penicillin-binding proteins (PBPs) located on the inner membrane of the bacterial cell wall. This binding inhibits the transpeptidation step of peptidoglycan synthesis, preventing the formation of cross-links that give the bacterial cell wall its structural integrity. Without a functional cell wall, the bacterium undergoes osmotic lysis and dies. Cefixime is stable against many beta-lactamases produced by gram-negative organisms, which gives it a broader spectrum of activity compared to earlier generation cephalosporins.

DOSAGE AND ADMINISTRATION

·        Children older than 6 months and weighing less than 45kg: 8mg/kg/day given as a single daily dose or divided into two doses every 12 hours

·        Children weighing more than 45kg and adults: 400mg daily given as a single dose or divided into 200mg twice daily

·        For otitis media: 8mg/kg/day in divided doses is preferred over single daily dosing

·        Duration of therapy is typically 7 to 14 days depending on the infection type and severity

·        Shake the bottle thoroughly before measuring each dose using the provided measuring device

·        May be taken with or without food, though administration with food may reduce gastrointestinal side effects

SIDE EFFECTS

Common:

DRUG INTERACTIONS

Carbamazepine levels may increase when co-administered with cefixime, requiring monitoring. Antacids containing aluminum or magnesium hydroxide may reduce cefixime absorption and should be taken at least 2 hours apart. Probenecid delays renal excretion of cefixime, increasing plasma levels. Warfarin anticoagulant effect may be enhanced, requiring INR monitoring. Cefixime does not significantly interact with oral contraceptives at standard therapeutic doses.

STORAGE INSTRUCTIONS

Store the dry powder at room temperature between 20 to 25 degrees Celsius. After reconstitution with water, store the suspension in the refrigerator between 2 to 8 degrees Celsius.
