INDICATION

Maxef 2G Injection is the higher-strength formulation of ceftriaxone indicated for severe and life-threatening bacterial infections requiring maximum-dose cephalosporin therapy. These include bacterial meningitis, hospital-acquired pneumonia, complicated intra-abdominal infections, severe sepsis, febrile neutropenia, endocarditis, and Lyme neuroborreliosis. The 2g vial allows single-vial convenience for high-dose regimens, reducing preparation time and medication errors in critical care settings.

MECHANISM OF ACTION

Same bactericidal mechanism as Maxef 1G. Ceftriaxone 2g delivers a higher peak serum concentration, achieving greater bacterial kill in severe infections with higher organism burden or pathogens with elevated MICs. The long half-life of 6 to 9 hours maintains therapeutic drug levels above the MIC for the full 24-hour dosing interval, supporting time-dependent bacterial killing. The 2g dose is the standard for meningitis, endocarditis, and other deep-seated infections where tissue penetration is critical.

DOSAGE AND ADMINISTRATION

     Adults: 2g IV once daily for severe infections

     Bacterial meningitis: 2g IV every 12 hours (4g daily)

     Surgical prophylaxis: 2g IV as a single dose 30 to 60 minutes before incision

     Administer as a 30-minute IV infusion after reconstitution

     Reconstitute with sterile water or compatible diluent as per product insert

     Do not mix or co-infuse with calcium-containing solutions

     Dose adjustment is generally not required in renal impairment unless combined hepatic and renal failure is present

SIDE EFFECTS

Common:

     Injection site reactions including phlebitis with IV administration

     Diarrhea

     Nausea or vomiting

     Rash

Less common:

     Biliary sludge or gallbladder pseudolithiasis with prolonged high-dose therapy

     Pseudomembranous colitis

     Anaphylaxis in rare cases

     Hematological abnormalities including neutropenia, thrombocytopenia, and hemolytic anemia with prolonged use

     Pancreatitis in rare cases

NOT RECOMMENDED IN

     Known severe hypersensitivity to ceftriaxone, cephalosporins, or penicillins with documented cross-reactivity

     Neonates with jaundice or hyperbilirubinemia

     Concurrent IV calcium administration in neonates is absolutely contraindicated

     Severe combined hepatic and renal impairment without dose monitoring

DRUG INTERACTIONS

Same interactions as Maxef 1G. Absolute contraindication with calcium-containing IV solutions in neonates. Aminoglycosides should be administered separately. Warfarin monitoring is recommended. May cause false-positive urine glucose tests with copper reduction methods and false-positive Coombs test. Chloramphenicol may antagonize ceftriaxone's bactericidal effect.

STORAGE INSTRUCTIONS

Store below 25 degrees Celsius. Protect from light. After reconstitution, use within 6 hours at room temperature or 24 hours if refrigerated. Do not freeze reconstituted solution. Keep out of reach of children.
