INDICATION

Maxef 1G Injection is a third-generation cephalosporin antibiotic indicated for the treatment of a wide range of moderate to severe bacterial infections including lower respiratory tract infections such as community-acquired pneumonia, urinary tract infections, skin and soft tissue infections, bone and joint infections, intra-abdominal infections, bacterial meningitis, septicemia, and surgical prophylaxis. Its long half-life allows convenient once or twice daily dosing, making it one of the most widely prescribed injectable antibiotics in clinical practice.

MECHANISM OF ACTION

Ceftriaxone exerts bactericidal activity by binding to penicillin-binding proteins (PBPs) on the inner membrane of the bacterial cell wall, inhibiting the transpeptidation step of peptidoglycan synthesis. This disrupts cell wall integrity, leading to osmotic instability and bacterial lysis. Ceftriaxone has a broad spectrum of activity covering gram-positive organisms including Streptococcus pneumoniae and methicillin-susceptible Staphylococcus aureus, as well as gram-negative organisms including E. coli, Klebsiella, Proteus, Haemophilus influenzae, and Neisseria species. Its exceptionally long half-life of 6 to 9 hours allows effective once-daily dosing in most infections.

DOSAGE AND ADMINISTRATION

     Adults: 1 to 2g once daily by IV or deep IM injection

     For severe infections or meningitis: 2g once daily or up to 4g daily in divided doses

     Children: 50 to 80mg/kg once daily, maximum 4g daily

     Neonates: 20 to 50mg/kg once daily

     Reconstitute with sterile water for injection, 0.9% saline, or 1% lidocaine for IM injection

     IV administration: infuse over at least 30 minutes

     IM injection: inject deep into a large muscle mass such as the gluteal region

     Dose adjustment is not required in mild to moderate renal impairment due to dual hepatic and renal elimination

SIDE EFFECTS

Common:

     Injection site pain, particularly with IM administration

     Diarrhea or loose stools

     Nausea

Less common:

     Allergic reactions including rash, urticaria, or rarely anaphylaxis

     Pseudomembranous colitis with prolonged use

     Biliary sludge or pseudolithiasis, particularly in children and with prolonged therapy

     Elevated liver enzymes

     Eosinophilia or leukopenia

NOT RECOMMENDED IN

     Known hypersensitivity to ceftriaxone, other cephalosporins, or severe penicillin allergy with cross-reactivity

     Neonates with hyperbilirubinemia, especially premature neonates, due to risk of bilirubin displacement

     Do not reconstitute or mix with calcium-containing IV solutions in neonates due to fatal ceftriaxone-calcium precipitation risk

     History of gastrointestinal disease, especially antibiotic-associated colitis

DRUG INTERACTIONS

Ceftriaxone must never be mixed with calcium-containing IV solutions in neonates due to the risk of fatal precipitate formation in the lungs and kidneys. Aminoglycosides may have additive nephrotoxicity when used concurrently but should be administered at separate sites and times. Probenecid does not significantly affect ceftriaxone clearance due to its dual elimination pathway. Warfarin anticoagulant effect may be enhanced by disrupting Vitamin K-producing gut flora. High-dose ceftriaxone may cause false-positive Coombs test results.

STORAGE INSTRUCTIONS

Store the unreconstituted vial below 25 degrees Celsius. Protect from light. After reconstitution with sterile water, use within 6 hours at room temperature or 24 hours if refrigerated. Keep out of reach of children.
