INDICATION
Macdronic 5mg/100ml is a bisphosphonate indicated for the treatment of osteoporosis in postmenopausal women and men at high risk of fracture, treatment of Paget's disease of bone, and prevention of clinical fractures after hip fracture surgery. It is administered as a once-yearly intravenous infusion.
MECHANISM OF ACTION
Zoledronic acid binds to hydroxyapatite crystals on the bone surface, where it is taken up by osteoclasts during bone resorption. Inside osteoclasts, it inhibits farnesyl pyrophosphate synthase in the mevalonate pathway, blocking prenylation of small GTPases essential for osteoclast function and survival. This leads to osteoclast apoptosis, reducing bone resorption and increasing bone mineral density.
DOSAGE AND ADMINISTRATION
● 5mg administered as a single intravenous infusion over no less than 15 minutes
● For osteoporosis: once yearly
● For Paget's disease: single infusion; retreatment may be considered after 12 months if relapse occurs
● Ensure adequate hydration before and after infusion
● Supplement with calcium and Vitamin D throughout treatment
SIDE EFFECTS
Common:
● Acute phase reaction: fever, myalgia, headache, and fatigue within 3 days of infusion
● Mild hypocalcemia
Less common:
● Osteonecrosis of the jaw in rare cases, particularly in patients with dental disease or recent dental procedures
● Atypical femur fractures with long-term use
● Renal impairment with rapid infusion or in dehydrated patients
● Severe hypocalcemia in Vitamin D deficient patients
NOT RECOMMENDED IN
● Creatinine clearance below 35 ml/min
● Hypocalcemia (must be corrected before infusion)
● Known hypersensitivity to zoledronic acid or any bisphosphonate
● Pregnancy and breastfeeding
DRUG INTERACTIONS
Concurrent aminoglycosides may have additive hypocalcemic effect. NSAIDs may increase renal toxicity risk. Loop diuretics increase hypocalcemia risk. Ensure calcium and Vitamin D supplementation throughout treatment.
