INDICATION

Lumark Syrup 60ML is a broad-spectrum antiepileptic medication indicated as monotherapy for the treatment of partial-onset (focal) seizures with or without secondary generalization in patients aged 16 years and above, and as adjunctive therapy for partial-onset seizures in children aged 1 month and above, myoclonic seizures in patients aged 12 years and above with juvenile myoclonic epilepsy, and primary generalized tonic-clonic seizures in patients aged 6 years and above with idiopathic generalized epilepsy. The liquid formulation allows precise weight-based dosing in pediatric patients.

MECHANISM OF ACTION

Levetiracetam has a unique mechanism of action distinct from all other antiepileptic drugs. It binds to the synaptic vesicle protein SV2A, which is involved in the regulation of neurotransmitter vesicle fusion and exocytosis. By modulating SV2A function, levetiracetam reduces the excessive release of excitatory neurotransmitters from presynaptic neurons during seizure activity without affecting normal synaptic transmission. It does not interact with GABA receptors, sodium channels, or calcium channels at therapeutic doses, which contributes to its favorable side effect profile and minimal drug interaction potential.

DOSAGE AND ADMINISTRATION

●     Adults and adolescents above 50kg: starting dose 500mg twice daily (5ml twice daily), increased by 500mg twice daily every 2 to 4 weeks to a maximum of 1500mg twice daily

●     Children 6 months to 16 years: starting dose 10mg/kg twice daily, titrated up to 30mg/kg twice daily based on response

●     Infants 1 to 6 months: starting dose 7mg/kg twice daily, increased up to 21mg/kg twice daily

●     Use the provided measuring device for accurate dosing

●     May be taken with or without food

●     Swallow directly or dilute in a glass of water

●     Do not stop abruptly; taper gradually over at least 2 to 4 weeks to prevent breakthrough seizures

●     Dose reduction is required in patients with renal impairment

SIDE EFFECTS

Common:

●     Drowsiness or somnolence, particularly during the first few weeks

●     Fatigue and weakness

●     Headache

●     Dizziness

Less common:

●     Behavioral changes including irritability, agitation, mood swings, and hostility, particularly in children

●     Depression, anxiety, or emotional lability

●     Suicidal ideation, as with all antiepileptic medications

●     Decreased appetite or weight loss

●     Nasopharyngitis or upper respiratory infections

NOT RECOMMENDED IN

●     Known hypersensitivity to levetiracetam or any pyrrolidone derivative

●     Severe renal impairment without appropriate dose reduction

●     Patients with active suicidal ideation require close psychiatric monitoring

●     Do not discontinue abruptly; gradual tapering over at least 2 weeks is essential

DRUG INTERACTIONS

Levetiracetam has one of the cleanest drug interaction profiles among all antiepileptic medications. It does not undergo hepatic cytochrome P450 metabolism and does not induce or inhibit CYP enzymes. It does not interact with other antiepileptic drugs, oral contraceptives, warfarin, or digoxin at clinically meaningful levels. Probenecid may reduce the renal clearance of the primary inactive metabolite but does not affect levetiracetam levels. Methotrexate clearance may be slightly reduced with concurrent use.
