INDICATION
Loprot 40mg Injection is a proton pump inhibitor indicated for the treatment of conditions where rapid acid suppression is required, including erosive esophagitis, bleeding peptic ulcers, Zollinger-Ellison syndrome, and stress ulcer prophylaxis in critically ill patients who cannot take oral medication.
MECHANISM OF ACTION
Omeprazole irreversibly inhibits the hydrogen-potassium ATPase enzyme system (proton pump) on the parietal cells of the gastric mucosa. This blocks the final step of acid secretion, reducing both basal and stimulated gastric acid output by up to 95%. IV administration achieves rapid plasma levels and immediate acid suppression in patients unable to swallow oral formulations.
DOSAGE AND ADMINISTRATION
● Adults: 40mg IV once daily as a slow injection over at least 2.5 minutes or as a 20 to 30 minute infusion
● For bleeding ulcers: 80mg IV bolus followed by continuous infusion of 8mg/hour for 72 hours
● Reconstitute with the provided diluent before administration
● Administer within 12 hours of reconstitution if stored at room temperature
● Switch to oral therapy as soon as the patient can tolerate it
SIDE EFFECTS
Common:
● Headache
● Injection site reactions
● Nausea or flatulence
Less common:
● Hypomagnesemia with prolonged use
● Clostridium difficile-associated diarrhea
● Increased fracture risk with long-term high-dose therapy
● Vitamin B12 deficiency with chronic use
NOT RECOMMENDED IN
● Known hypersensitivity to omeprazole or other proton pump inhibitors
● Concurrent use with rilpivirine or atazanavir
● Hepatic impairment requires dose adjustment
DRUG INTERACTIONS
Omeprazole inhibits CYP2C19 and may increase levels of diazepam, phenytoin, and clopidogrel (reducing its antiplatelet effect). Rilpivirine and atazanavir require gastric acid for absorption and are contraindicated with PPIs. Methotrexate clearance may be reduced, increasing toxicity risk.
