INDICATION

Listono 1 million IU Inj is a polymyxin antibiotic indicated for the treatment of serious and life-threatening infections caused by multidrug-resistant (MDR) gram-negative bacteria, including Pseudomonas aeruginosa, Acinetobacter baumannii, and Klebsiella pneumoniae. It is used when other antibiotics have failed or are not suitable, particularly in hospital-acquired pneumonia, bacteremia, urinary tract infections, and sepsis caused by carbapenem-resistant organisms.

MECHANISM OF ACTION

Colistimethate sodium is an inactive prodrug that is hydrolyzed in vivo to colistin, the active polymyxin compound. Colistin acts by binding to lipopolysaccharide (LPS) and phospholipids in the outer membrane of gram-negative bacteria, displacing divalent cations and disrupting membrane integrity. This increases membrane permeability, causing leakage of intracellular contents and rapid bactericidal cell death. Because of its unique mechanism targeting the bacterial outer membrane, colistin retains activity against many organisms resistant to all other antibiotic classes.

DOSAGE AND ADMINISTRATION

•      Adults: 2.5 to 5 mg/kg/day of colistin base activity in 2 to 4 divided doses by IV infusion

•      Loading dose recommended in critically ill patients: 9 million IU as a single infusion

•      Dose must be adjusted based on renal function; reduced doses mandatory in renal impairment

•      Administered only by intravenous infusion in a hospital or ICU setting under strict medical supervision

SIDE EFFECTS

Common:

•      Nephrotoxicity: elevated creatinine, reduced urine output (monitor renal function closely)

•      Neurotoxicity: dizziness, tingling, slurred speech, neuromuscular blockade

•      Injection site reactions with peripheral administration

•      Nausea and vomiting

Less common:

•      Acute kidney injury requiring dialysis with prolonged high-dose use

•      Respiratory failure due to neuromuscular blockade at high doses

•      Bronchospasm with inhaled formulations

•      Hypersensitivity reactions including rash and anaphylaxis

DRUG INTERACTIONS

Concurrent use of aminoglycosides, vancomycin, amphotericin B, or other nephrotoxic agents significantly increases the risk of acute kidney injury; avoid combination or monitor renal function very closely. Neuromuscular blocking agents used with colistin increase the risk of prolonged paralysis and respiratory failure; use with extreme caution in surgical patients. Non-depolarizing muscle relaxants may have enhanced and prolonged effects when combined with colistin. Loop diuretics such as furosemide may potentiate nephrotoxicity.
