INDICATION

Lepitam 250mg Tab is an antiepileptic drug (AED) used as adjunctive therapy in adults and children with partial-onset seizures, with or without secondary generalization. It is also indicated for myoclonic seizures in juvenile myoclonic epilepsy and for primary generalized tonic-clonic seizures.

MECHANISM OF ACTION

Levetiracetam's precise mechanism of action is not fully understood but is distinctly different from other antiepileptics. It binds selectively to synaptic vesicle protein 2A (SV2A) in the brain, a protein involved in synaptic vesicle exocytosis and neurotransmitter release. This binding modulates vesicle fusion and reduces excessive neuronal firing without broadly affecting normal synaptic transmission. Levetiracetam also inhibits N-type calcium channels and reduces the activity of GABA-negative allosteric modulators, contributing to its anticonvulsant profile.

DOSAGE AND ADMINISTRATION

•      Adults: initial dose 250mg twice daily; increase by 500mg every 2 weeks as tolerated

•      Maintenance dose: 500mg to 1500mg twice daily

•      Dose reduction required in renal impairment (dose adjusted per creatinine clearance)

•      Always follow the dosage and titration schedule prescribed by your neurologist

SIDE EFFECTS

Common:

•      Somnolence (drowsiness)

•      Dizziness and headache

•      Weakness and fatigue

Less common:

DRUG INTERACTIONS

It does not induce or inhibit cytochrome P450 enzymes. However, probenecid may reduce renal elimination of the active metabolite. CNS depressants including alcohol may potentiate sedative effects.
