INDICATION

Lacolep 150mg is an antiepileptic medication indicated as monotherapy or adjunctive therapy for the treatment of partial-onset (focal) seizures with or without secondary generalization in adults and adolescents aged 16 years and above with epilepsy.

MECHANISM OF ACTION

Lacosamide selectively enhances the slow inactivation of voltage-gated sodium channels, stabilizing hyperexcitable neuronal membranes without affecting normal neuronal firing. This distinguishes it from traditional sodium channel blockers like carbamazepine and phenytoin, which primarily affect fast inactivation. By targeting slow inactivation, lacosamide reduces the availability of sodium channels for repetitive firing during seizure activity while preserving normal brain function.

DOSAGE AND ADMINISTRATION

●     Adults: starting dose 50mg twice daily, titrated by 50mg twice daily at weekly intervals

●     Recommended maintenance: 150 to 200mg twice daily

●     Maximum: 400mg daily (200mg twice daily)

●     Swallow tablets whole with water

●     May be taken with or without food

●     Do not discontinue abruptly; taper over at least one week

SIDE EFFECTS

Common:

●     Dizziness

●     Headache

●     Nausea

●     Diplopia (double vision)

Less common:

●     PR interval prolongation on ECG

●     Ataxia or balance disturbance

●     Suicidal ideation, as with all antiepileptics

●     Multiorgan hypersensitivity reaction in rare cases

DRUG INTERACTIONS

PR-prolonging drugs including beta-blockers and calcium channel blockers increase the risk of cardiac conduction abnormalities. Strong enzyme inducers like rifampicin, carbamazepine, and phenytoin may reduce lacosamide plasma levels.
