INDICATION:

 Kisqali 200mg is indicated for adults with hormone receptor (HR)-positive, HER2-negative advanced or metastatic breast cancer, used in combination with an aromatase inhibitor or fulvestrant. It is also approved as adjuvant therapy for HR-positive, HER2-negative stage II and III early breast cancer at high risk of recurrence, again in combination with an aromatase inhibitor. Kisqali helps slow tumor progression, reduce recurrence risk, and improve patient survival. The therapy is tailored by oncologists and often monitored alongside hormonal treatments. It is used in carefully scheduled cycles to maximize efficacy while monitoring tolerability.

MECHANISM OF ACTION:

 Ribociclib is a selective cyclin-dependent kinase 4 and 6 (CDK4/6) inhibitor. In HR-positive breast cancer, CDK4/6 drives progression from the G1 to S phase of the cell cycle by phosphorylating the retinoblastoma protein (pRb). Ribociclib inhibits CDK4/6 activity, blocking pRb phosphorylation, which arrests cancer cells in the G1 phase. This inhibits tumor cell proliferation, reduces tumor growth, and enhances the efficacy of concurrent hormonal therapy.

DOSAGE AND ADMINISTRATION:

●     Adults: 600mg (three 200mg tablets) once daily for 21 consecutive days, followed by 7 days off, in a 28-day cycle.

●     Adjuvant early breast cancer: 400mg daily in the same 21/7 schedule for up to 3 years.

●     Take tablets at the same time each morning, with or without food.

●     Do not crush or split tablets.

●     Dose adjustments may be needed based on toxicity or adverse reactions.

 

SIDE EFFECTS:

●     Common: neutropenia, nausea, fatigue, diarrhea, alopecia, headache, constipation, infections.

●     Serious: severe neutropenia, QTc prolongation, hepatotoxicity, interstitial lung disease/pneumonitis.

●     Monitoring: regular blood counts, liver function tests, and ECG are mandatory.

NOT RECOMMENDED IN:
