INDICATION

Kestine 5mg/5ml Syrup is a second-generation non-sedating antihistamine indicated for the symptomatic relief of allergic rhinitis (both seasonal and perennial), allergic conjunctivitis, and chronic idiopathic urticaria in adults and children.

MECHANISM OF ACTION

Ebastine is a selective peripheral H1 histamine receptor antagonist. It is rapidly metabolized to its active metabolite carebastine, which competitively blocks H1 receptors on vascular endothelium, smooth muscle, and sensory nerves. This prevents histamine-mediated vasodilation, increased vascular permeability, itching, and bronchoconstriction. Unlike first-generation antihistamines, ebastine has minimal blood-brain barrier penetration and therefore causes negligible sedation.

DOSAGE AND ADMINISTRATION

●     Adults and children above 12 years: 10ml (10mg) once daily

●     Children 6 to 12 years: 5ml (5mg) once daily

●     Children 2 to 6 years: 2.5ml (2.5mg) once daily or as directed

●     May be taken with or without food

●     Shake well before use

SIDE EFFECTS

Common:

●     Headache

●     Dry mouth

●     Mild drowsiness in sensitive individuals

Less common:

●     QTc prolongation at doses exceeding recommended limits

●     Abdominal pain in rare cases

●     Allergic reactions in very rare cases

NOT RECOMMENDED IN

●     Known hypersensitivity to ebastine or any component

●     Severe hepatic impairment

●     Concurrent use with potent CYP3A4 inhibitors at elevated doses

DRUG INTERACTIONS

CYP3A4 inhibitors such as ketoconazole and erythromycin increase ebastine plasma levels and QTc prolongation risk at doses above 20mg. Avoid concurrent use with other QTc-prolonging medications. No significant interaction at standard doses.
