INDICATION

Inograf 1mg Cap is an immunosuppressant indicated for the prophylaxis of organ rejection in patients who have received kidney, liver, or heart transplants. It is used as part of combination immunosuppressive regimens to prevent the body's immune system from attacking the transplanted organ.

MECHANISM OF ACTION

Tacrolimus is a calcineurin inhibitor that binds to an intracellular immunophilin protein called FK-binding protein 12 (FKBP-12). This drug-protein complex inhibits calcineurin, a phosphatase required for the activation of the nuclear factor of activated T-cells (NFAT). Blocking NFAT prevents the transcription of interleukin-2 and other cytokines essential for T-lymphocyte activation and proliferation, thereby suppressing the immune response against the transplanted organ.

DOSAGE AND ADMINISTRATION

      Dosage is highly individualized based on transplant type, body weight, and through blood levels

      Initial adult kidney transplant dose: typically, 0.2 to 0.3mg/kg/day in two divided doses

      Regular therapeutic drug monitoring (trough levels) is mandatory

      Always prescribed and monitored by a transplant specialist; never self-adjust the dose

SIDE EFFECTS

Common:

      Tremor

      Headache and insomnia

      Hyperglycemia and new-onset diabetes

      Hypertension

Less common:

      Nephrotoxicity (dose-dependent; monitor renal function)

      Neurotoxicity: seizures, leukoencephalopathy

      QT prolongation and cardiac arrhythmias

      Increased susceptibility to infections and malignancies with prolonged use

NOT RECOMMENDED IN

      Concurrent use with cyclosporine (additive nephrotoxicity)

      Known hypersensitivity to tacrolimus or polyoxyl 60 hydrogenated castor oil

      Pregnancy and breastfeeding (unless benefit outweighs risk under specialist guidance)

      Grapefruit and grapefruit juice must be avoided

DRUG INTERACTIONS

Strong CYP3A4 inhibitors including azole antifungals, macrolide antibiotics, and protease inhibitors markedly increase tacrolimus blood levels, raising toxicity risk; dose reduction and frequent monitoring required. CYP3A4 inducers such as rifampicin and St John's Wort reduce tacrolimus levels, risking rejection. NSAIDs combined with tacrolimus increase nephrotoxicity. Potassium-sparing diuretics increase hyperkalemia risk.
