INDICATION

Indaz 500mg Tab is a macrolide antibiotic indicated for the treatment of mild to moderate bacterial infections caused by susceptible organisms. These include community-acquired pneumonia, acute bacterial exacerbations of chronic bronchitis, pharyngitis, tonsillitis, sinusitis, skin and soft tissue infections, and pelvic inflammatory disease. The 500mg strength is used as the standard loading dose or for conditions requiring higher azithromycin concentrations.

MECHANISM OF ACTION

Azithromycin binds reversibly to the 50S ribosomal subunit of susceptible bacteria, blocking translocation of peptides and inhibiting RNA-dependent protein synthesis. It is bacteriostatic at standard doses but may be bactericidal at high concentrations against certain organisms. Its exceptionally long tissue half-life allows once-daily dosing and short treatment courses, as drug concentrations remain therapeutic at the site of infection for several days after the last dose.

DOSAGE AND ADMINISTRATION

•      Community-acquired pneumonia: 500mg once daily on day 1, then 250mg once daily on days 2 to 5

•      Sinusitis and skin infections: 500mg once daily for 3 days

•      Pelvic inflammatory disease: 500mg IV or oral once daily for 1 to 2 days, then 250mg once daily to complete 7 days

•      Always complete the full prescribed course even if symptoms improve early

SIDE EFFECTS

Common:

•      Nausea, vomiting, diarrhoea

•      Abdominal pain and stomach discomfort

•      Headache

•      Dizziness

Less common:

•      QT prolongation and cardiac arrhythmias (rare; caution in at-risk patients)

•      Clostridium difficile-associated diarrhoea with prolonged use

•      Abnormal liver function tests or cholestatic jaundice

•      Hypersensitivity reactions including rash, urticaria, and angioedema

NOT RECOMMENDED IN

•      Known hypersensitivity to azithromycin, erythromycin, or other macrolide antibiotics

•      History of cholestatic jaundice or hepatic dysfunction with prior azithromycin use

•      Patients at risk of QT prolongation or taking other QT-prolonging drugs

•      Severe hepatic impairment

DRUG INTERACTIONS

Azithromycin can prolong the QT interval; avoid concurrent use with other QT-prolonging agents including antiarrhythmics, fluoroquinolones, and antipsychotics. Antacids containing Aluminium or magnesium reduce the rate of azithromycin absorption; take azithromycin at least 1 hour before or 2 hours after antacids. Warfarin anticoagulant effect may be enhanced; monitor INR closely during and after therapy. Cyclosporine levels may be increased; monitor drug levels and renal function.
