INDICATION

Iboxime 400mg Cap is a third-generation oral cephalosporin antibiotic indicated for the treatment of mild to moderate infections caused by susceptible bacteria. These include urinary tract infections, uncomplicated gonorrhea, pharyngitis and tonsillitis caused by Streptococcus pyogenes, and acute exacerbations of chronic bronchitis.

MECHANISM OF ACTION

Cefixime exerts its bactericidal effect by binding to penicillin-binding proteins in the bacterial cell wall, inhibiting peptidoglycan synthesis. This disrupts cell wall formation and leads to bacterial lysis. Cefixime is resistant to many beta-lactamases, giving it activity against a wide range of gram-negative organisms. It has excellent oral bioavailability compared to other cephalosporins and achieves adequate serum and urinary concentrations for treating common infections.

DOSAGE AND ADMINISTRATION

•      Adults: 400mg once daily or 200mg twice daily

•      Uncomplicated gonorrhea: 400mg as a single dose

•      Take with or without food; food may reduce stomach irritation

•      Always complete the full prescribed course as directed by your doctor

SIDE EFFECTS

Common:

•      Diarrhoea (most common side effect)

•      Nausea and abdominal discomfort

•      Headache

•      Skin rash or urticaria

Less common:

•      Clostridium difficile colitis with prolonged use

•      Eosinophilia

•      Elevated liver enzymes

•      Hypersensitivity reactions including angioedema

NOT RECOMMENDED IN

•      Known hypersensitivity to cephalosporins or penicillin’s (cross-reactivity possible)

•      Severe renal impairment (dose reduction required)

•      Not recommended as sole treatment for penicillinase-producing Staphylococcus infections

•      Caution in patients with history of gastrointestinal disease

DRUG INTERACTIONS

Probenecid increases and prolongs cefixime plasma levels by reducing renal tubular secretion. Anticoagulants such as warfarin may have enhanced effect with cefixime; monitor INR. Nifedipine has been reported to increase cefixime bioavailability. Antacids containing Aluminium or magnesium may slightly reduce absorption; take separately.
