INDICATION
Hypozam 5mg Injection is a short-acting benzodiazepine indicated for preoperative sedation, conscious sedation for diagnostic and therapeutic procedures, induction of general anesthesia, sedation in intensive care units, and management of status epilepticus when first-line treatments have failed.
MECHANISM OF ACTION
Midazolam enhances the effect of gamma-aminobutyric acid (GABA) at the GABA-A receptor by binding to the benzodiazepine site on the receptor complex. This increases the frequency of chloride channel opening, causing neuronal hyperpolarization and central nervous system depression. The result is sedation, anxiolysis, amnesia, muscle relaxation, and anticonvulsant activity. Midazolam has a rapid onset (1 to 5 minutes IV) and short duration (20 to 60 minutes), making it ideal for procedural sedation.
DOSAGE AND ADMINISTRATION
● Administered intravenously, intramuscularly, or intranasally under medical supervision only
● IV sedation: 0.5 to 2.5mg slowly over 2 minutes, titrated to effect
● IM premedication: 0.07 to 0.1mg/kg, 20 to 30 minutes before the procedure
● Status epilepticus: 0.15 to 0.2mg/kg IV or IM
● Dose must be individualized and reduced in elderly, debilitated, or hepatically impaired patients
● Resuscitation equipment must be immediately available during administration
SIDE EFFECTS
Common:
● Drowsiness and sedation
● Respiratory depression, particularly with rapid IV administration
● Temporary anterograde amnesia
Less common:
● Paradoxical reactions including agitation and restlessness in rare cases
● Hypotension with rapid injection
● Laryngospasm in very rare cases
● Physical dependence with prolonged use
NOT RECOMMENDED IN
● Severe respiratory insufficiency
● Acute narrow-angle glaucoma
● Known hypersensitivity to benzodiazepines
● Myasthenia gravis
● Concurrent use with potent opioids without monitoring
DRUG INTERACTIONS
Opioids, other CNS depressants, and alcohol significantly increase the risk of respiratory depression, profound sedation, and death. CYP3A4 inhibitors such as ketoconazole, erythromycin, and ritonavir increase midazolam plasma levels. Rifampicin reduces midazolam levels through CYP3A4 induction. Concurrent use with protease inhibitors is contraindicated.
