INDICATION

Hydrourea 500mg Capsules are used in the treatment of sickle cell disease to reduce the frequency of painful crises and the need for blood transfusions. It is also indicated in certain myeloproliferative disorders including chronic myeloid leukemia (CML), polycythemia vera, and essential thrombocythemia.

MECHANISM OF ACTION

Hydroxyurea is an antineoplastic and anti-sickle cell agent that inhibits ribonucleotide reductase, an enzyme required for the conversion of ribonucleotides to deoxyribonucleotides, which are essential for DNA synthesis. This blocks the S phase of the cell cycle and results in cytoreduction. In sickle cell disease, hydroxyurea increases fetal hemoglobin (HbF) production, which dilutes the polymerizing HbS and reduces sickling, vaso-occlusion, and hemolysis.

DOSAGE AND ADMINISTRATION

•      Sickle cell disease: initial dose 15mg/kg/day, titrated to response and toxicity

•      Myeloproliferative disorders: 20 to 30mg/kg/day initially

•      Dose adjustments based on blood counts; requires regular monitoring

•      Always prescribed and managed by a hematologist; do not self-adjust dose

SIDE EFFECTS

Common:

•      Bone marrow suppression: anemia, leucopenia, thrombocytopenia

•      Nausea and vomiting

•      Oral mucositis and mouth ulcers

•      Skin rash and leg ulcers with long-term use

Less common:

•      Pulmonary toxicity

•      Elevated creatinine (renal toxicity)

•      Peripheral neuropathy

•      Secondary leukemia with prolonged use (rare)

Regular blood count monitoring is essential to detect early toxicity.
