INDICATION
Hizone 1G Inj is a third-generation cephalosporin antibiotic used to treat serious bacterial infections including community-acquired pneumonia, meningitis, pelvic inflammatory disease, septicemia, surgical prophylaxis, intra-abdominal infections, skin and soft tissue infections, and urinary tract infections caused by susceptible organisms.
MECHANISM OF ACTION
Ceftriaxone is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), which are enzymes responsible for cross-linking peptidoglycan chains in the bacterial cell wall. This binding blocks transpeptidation, causing defects in the cell wall and ultimately bacterial lysis. Ceftriaxone has excellent stability against many beta-lactamases, a long half-life allowing once-daily dosing, and good penetration into the cerebrospinal fluid, making it effective for CNS infections.
DOSAGE AND ADMINISTRATION
• Adults: 1 to 2g once daily by intravenous or intramuscular injection
• Meningitis: 2g every 12 hours initially
• Gonorrhea: 250mg to 500mg single intramuscular dose
• Always administered by a healthcare professional; dosage adjusted based on infection severity and renal function
SIDE EFFECTS
Common:
• Pain and tenderness at injection site
• Diarrhoea
• Nausea
• Elevated liver enzymes (transaminases)
Less common:
• Clostridium difficile-associated diarrhoea with prolonged use
• Biliary sludge or pseudolithiasis (particularly in children)
• Thrombophlebitis with intravenous use
• Hypersensitivity reactions including rash, urticaria, or anaphylaxis
NOT RECOMMENDED IN
• Known hypersensitivity to cephalosporins or penicillin’s (cross-reactivity possible)
• Premature neonates and neonates with jaundice or hyperbilirubinemia (risk of bilirubin displacement)
• Do not mix with calcium-containing IV solutions in any patient
• Not for self-administration; hospital or clinic use only
DRUG INTERACTIONS
Concurrent use with aminoglycoside antibiotics such as gentamicin increases nephrotoxicity risk; monitor renal function closely. Ceftriaxone forms a potentially fatal precipitate with calcium-containing solutions, including Ringer's lactate; never mix. Probenecid may reduce renal excretion of ceftriaxone, increasing its plasma levels. Vitamin K antagonists such as warfarin may have enhanced anticoagulant effects with ceftriaxone.
